Grades of Prostatomegaly and Best Diagnostic Method
Digital rectal examination (DRE) combined with transrectal ultrasonography (TRUS) is the best method to diagnose prostatomegaly, with prostate volume classified as mild (<20 mL), moderate (<50 mL), or severe (≥50 mL). 1, 2
Grading System for Prostatomegaly
The anatomic severity of prostatomegaly is graded based on prostate volume measured by TRUS 2:
- Mild: <20 mL
- Moderate: <50 mL
- Severe: ≥50 mL
This grading system was validated through multi-institutional studies involving 692 BPH patients and demonstrated approximately 80% accuracy in predicting physician assessment of disease severity 2.
Diagnostic Approach
Initial Physical Examination
DRE should be performed on all patients as the foundational diagnostic step 1. While DRE tends to underestimate true prostate size, if the prostate feels large on DRE, it is usually confirmed to be enlarged by ultrasound or other measurement techniques 1. This makes DRE a practical screening tool that can guide the need for further imaging.
Imaging for Objective Measurement
TRUS is the recommended imaging modality to objectively assess prostate size when treatment decisions depend on prostate volume 3. Obtaining TRUS imaging (or reviewing existing cross-sectional imaging) is reasonable to accurately measure prostate volume, particularly when considering 5-alpha reductase inhibitor (5-ARI) therapy 3.
A minimum prostate volume of >30cc is necessary for reliable 5-ARI response, though the larger the gland, the more pronounced the treatment effects 3. This threshold makes accurate volume measurement clinically important for treatment selection.
Clinical Context for Severity Assessment
Prostatomegaly severity should be evaluated across four domains, not just anatomy alone 2:
- Symptom severity: International Prostate Symptom Score (IPSS) with mild (0-7), moderate (8-19), severe (20-35) 3, 2
- Functional severity: Maximum flow rate and residual urine volume 2
- Anatomic severity: Prostate volume as described above 2
- Quality of life impact: QOL index scoring 2
Common Pitfalls
Do not rely on DRE alone for treatment planning. While DRE is adequate to exclude locally advanced cancer and provides initial assessment, objective measurement via TRUS is needed when prostate volume will influence therapeutic decisions, particularly for 5-ARI therapy or surgical planning 3, 1.
PSA levels correlate with prostate volume and should be measured in select patients—those with at least 10-year life expectancy where prostate cancer knowledge would change management, or when PSA may influence voiding symptom management 1. A PSA >1.5 ng/mL suggests adequate gland size for 5-ARI response 3.
