Daratumumab-VRd Weekly Dosing Schedule for First 4-Week Cycle
For the first four-week cycle of daratumumab combined with bortezomib, lenalidomide, and dexamethasone (Dara-VRd), daratumumab 16 mg/kg IV should be administered weekly (on days 1,8,15, and 22), while bortezomib 1.3 mg/m² is given subcutaneously on days 1,4,8, and 11, lenalidomide 25 mg orally on days 1-14, and dexamethasone 20 mg on the day of and day after bortezomib (or 40 mg on days 1,8,15,22). 1
Detailed Dosing Algorithm for Cycle 1 (4 Weeks)
Daratumumab Administration
- Week 1 (Days 1,8): 16 mg/kg IV weekly 1
- Week 2 (Day 15): 16 mg/kg IV 1
- Week 3 (Day 22): 16 mg/kg IV 1
- Total: 4 doses in the first 4-week cycle 1
Bortezomib Administration
Lenalidomide Administration
- Days 1-14: 25 mg orally daily 2
- Note: This differs from the 3-week VRd schedule where lenalidomide is given days 1-14 of a 21-day cycle 2
Dexamethasone Administration
Critical Dosing Distinctions
The key difference between standard VRd and Dara-VRd is the cycle length and daratumumab dosing frequency. Standard VRd without daratumumab uses 3-week cycles with bortezomib on days 1,8,15 only 2. However, when daratumumab is added, the regimen transitions to 4-week cycles with more intensive bortezomib dosing (days 1,4,8,11) during the first 8 cycles 1, 3.
Subsequent Cycles (After Cycle 1)
- Cycles 2-8: Continue weekly daratumumab (total 8 doses per cycle 1-8) 1
- Cycles 9-24: Daratumumab every 2 weeks 1
- Cycle 25 onwards: Daratumumab every 4 weeks until progression 1
Pre-Medication Requirements
Administer 1-3 hours before each daratumumab infusion: 1
- Dexamethasone 20 mg IV or PO (or equivalent corticosteroid)
- Antihistamine (oral or IV)
- Antipyretic (oral acetaminophen 650-1000 mg)
Common Pitfalls to Avoid
Do not confuse the 3-week VRd cycle schedule with the 4-week Dara-VRd schedule – this is a frequent error that can lead to underdosing of bortezomib 2, 1. The addition of daratumumab necessitates the 4-week cycle structure with twice-weekly bortezomib during cycles 1-8 1, 3.
Monitor for increased peripheral neuropathy risk with the twice-weekly bortezomib schedule, particularly in weeks 1-2 of each cycle 4, 3. Grade ≥2 peripheral neuropathy occurred in 27% of patients receiving weekly subcutaneous bortezomib with isatuximab-VRd 4.
Evidence Supporting This Regimen
The GRIFFIN trial demonstrated that Dara-VRd significantly improved stringent complete response rates (67% vs 48%) and 4-year progression-free survival (87.2% vs 70.0%) compared to VRd alone in transplant-eligible patients 3. The CEPHEUS trial confirmed similar benefits in transplant-ineligible patients, with MRD-negativity rates of 60.9% versus 39.4% with VRd alone 5.
