Protonix (Pantoprazole) Dosing Recommendations

Standard Adult Dosing

For most gastric acid-related disorders in adults, the optimal dose is pantoprazole 40 mg once daily. 1, 2

  • Erosive Esophagitis (EE) associated with GERD: 40 mg once daily for up to 8 weeks 1
  • Maintenance of healed EE: 40 mg once daily 1
  • Pathological hypersecretory conditions: Initial dose 40 mg twice daily, with doses up to 240 mg/day used in some patients 1

Pediatric Dosing

Children 5 Years and Older

For short-term treatment of EE associated with GERD in children ≥5 years:

  • Weight 15 kg to <40 kg: 20 mg once daily for up to 8 weeks 1
  • Weight ≥40 kg: 40 mg once daily for up to 8 weeks 1

The FDA label specifically states that for patients less than 5 years of age, there is no appropriate dosage strength in an age-appropriate formulation available, despite clinical trial data supporting efficacy in younger children 1.

Children 1 to 5 Years (Off-Label, Based on Clinical Trials)

  • Dose: Approximately 1.2 mg/kg once daily (15 mg for ≤12.5 kg; 20 mg for >12.5 to <25 kg) 1
  • Duration: 8 weeks for treatment of EE 1

Infants Less Than 1 Year

Pantoprazole is NOT indicated for infants less than 1 year of age. 1 A randomized, placebo-controlled trial demonstrated no efficacy in this age group, and the FDA label explicitly states that effectiveness has not been demonstrated in patients less than 1 year of age 1.

Eosinophilic Esophagitis (EoE) Dosing

Adults with EoE

Omeprazole 20 mg twice daily for at least 8-12 weeks is the recommended PPI regimen for EoE, as it is the only PPI assessed for this indication. 3 However, when extrapolating to pantoprazole:

  • Dose: 20-40 mg twice daily for 8-12 weeks 4
  • The British Society of Gastroenterology (2022) specifically recommends omeprazole 20 mg twice daily as the evidence-based choice for EoE 3

Pediatric Patients with EoE

  • Dose: 1 mg/kg twice daily (maximum 40 mg twice daily) for 8-12 weeks 3, 4, 5
  • Repeat endoscopy with biopsies should be performed while on treatment to assess histological response 3

Helicobacter pylori Eradication

For H. pylori eradication, pantoprazole is used as part of combination therapy:

  • Standard regimen: Pantoprazole 40 mg twice daily + amoxicillin 1000 mg twice daily + clarithromycin 500 mg twice daily for 10-14 days 6
  • The dose should be taken 30 minutes before meals for optimal acid suppression 6, 5

Administration Timing

  • Delayed-release tablets: May be taken without regard to meals, though food may delay absorption by 2 hours without affecting overall bioavailability 1
  • Oral suspension (granules): Should be taken approximately 30 minutes before a meal, as high-fat meals decrease Cmax by 51% and AUC by 29% 1
  • For H. pylori eradication or EoE: Administer 30 minutes before morning and evening meals for twice-daily dosing 5

Special Populations

Hepatic Impairment

  • No dose adjustment required for mild to severe hepatic impairment (Child-Pugh A to C), though doses higher than 40 mg/day have not been studied in this population 1

Renal Impairment

  • No dose adjustment required in patients with severe renal impairment 1

Geriatric Patients

  • No dose adjustment required, though slight increases in AUC (43%) and Cmax (26%) have been observed in elderly patients 1

Important Clinical Caveats

Do not use pantoprazole in infants less than 1 year of age – a placebo-controlled trial showed no efficacy, and prolonged PPI use in infants is associated with increased risk of lower respiratory tract infections 5, 1, 7.

For EoE management, do not reduce to lower maintenance doses in primary care, as the therapeutic effect requires sustained high-dose PPI therapy 3.

Twice-daily dosing is superior to once-daily dosing for EoE and PPI-responsive esophageal eosinophilia, with a non-significant trend toward increased efficacy demonstrated in meta-analysis 3.

Long-term safety data extending up to 15 years demonstrate that pantoprazole maintenance therapy is effective and well-tolerated with no identified safety concerns in adults with severe acid-peptic disease 8.
