Tolvaptan: Indications and Contraindications

Tolvaptan is indicated for slowing progression of ADPKD in adults with CKD stages 1–3 who have rapidly progressing disease, and for treating clinically significant hypervolemic or euvolemic hyponatremia (serum sodium <125 mEq/L or symptomatic hyponatremia resistant to fluid restriction) in heart failure and SIADH—but it is absolutely contraindicated in ADPKD patients outside the FDA-approved REMS program, in patients unable to sense or respond to thirst, in hypovolemic hyponatremia, with concurrent strong CYP3A inhibitors, in anuria, and in those with hypersensitivity to the drug. 1, 2

Primary Indications

For Autosomal Dominant Polycystic Kidney Disease (ADPKD)

  • Tolvaptan slows kidney disease progression in ADPKD patients with CKD stages 1–3 at treatment initiation who demonstrate rapid disease progression. 1
  • Evidence from the TEMPO 3:4 trial showed tolvaptan reduced eGFR decline by 1.3 mL/min/1.73 m² per year and total kidney volume growth by 2.7% compared to placebo. 1
  • The REPRISE trial confirmed efficacy in later-stage ADPKD (CKD stage 4), allowing continuation in patients aged >55 years or with eGFR <25 mL/min/1.73 m² if well tolerated. 1
  • Tolvaptan also reduced the incidence of UTI, kidney stones, hematuria, and kidney pain events in ADPKD patients. 1

For Hyponatremia

  • Tolvaptan is indicated for clinically significant hypervolemic and euvolemic hyponatremia (serum sodium <125 mEq/L or symptomatic hyponatremia that has resisted fluid restriction) in patients with heart failure and SIADH. 2
  • It is effective in raising serum sodium in patients with heart failure, cirrhosis, and SIADH through aquaresis (electrolyte-free water excretion). 3, 4
  • Tolvaptan should NOT be used for patients requiring urgent correction of serum sodium to prevent or treat serious neurological symptoms—hypertonic saline is preferred in acute, severe, symptomatic hyponatremia. 2, 5

Absolute Contraindications

Regulatory and Safety-Based Contraindications

  • ADPKD patients outside the FDA-approved REMS program are absolutely contraindicated from receiving tolvaptan due to hepatotoxicity risk. 2
  • Patients unable to sense or respond to thirst cannot safely compensate for the obligatory aquaresis induced by tolvaptan. 2
  • Hypovolemic hyponatremia is an absolute contraindication because tolvaptan causes further water loss without addressing the underlying volume deficit. 2, 5
  • Concurrent use of strong CYP3A inhibitors (e.g., itraconazole, ketoconazole, clarithromycin, ritonavir) is contraindicated because they reduce tolvaptan clearance by >80%, leading to excessive drug levels. 6, 2
  • Anuria precludes any therapeutic effect of tolvaptan, which requires renal water excretion. 2
  • Hypersensitivity to tolvaptan (e.g., anaphylactic shock, generalized rash) is an absolute contraindication. 2

Relative Contraindications and Situations Requiring Treatment Interruption

Volume Depletion and "Sick-Day" Scenarios

  • Tolvaptan must be interrupted during any situation causing volume depletion or inability to compensate for aquaresis, including vomiting, diarrhea, fever, limited water access, or prolonged exposure to warm environments. 1, 6
  • Patients should have a "sick-day plan" and skip all doses during these periods to prevent hypovolemia. 1, 6

Drug Interactions

  • Moderate CYP3A inhibitors (e.g., fluconazole, erythromycin, diltiazem, verapamil, grapefruit juice) should be avoided because they reduce tolvaptan clearance by 50–80%. 1, 6
  • Dose reduction or temporary discontinuation is required when moderate CYP3A inhibitors cannot be avoided. 1, 6

Hepatotoxicity Risk

  • Tolvaptan should be held if ALT or AST rises to ≥3× upper limit of normal (ULN) or >2× baseline, or if ALT/AST exceeds 2× ULN with hepatic symptoms (fatigue, nausea, vomiting, right upper quadrant pain, jaundice, pruritus). 6
  • Approximately 5% of ADPKD patients on tolvaptan develop transaminase elevations >3× ULN (vs. 1% on placebo), typically within the first 18 months. 1, 6
  • Permanently discontinue tolvaptan if elevations persist at ≥3× ULN without an alternative explanation after 48–72 hours. 6

Critical Warnings and Monitoring Requirements

Risk of Osmotic Demyelination Syndrome

  • Too rapid correction of hyponatremia (>12 mEq/L per 24 hours) can cause osmotic demyelination syndrome, manifesting as dysarthria, dysphagia, seizures, lethargy, spastic quadriparesis, coma, or death. 6, 2
  • In controlled trials, 7% of tolvaptan-treated subjects with baseline sodium <130 mEq/L had increases >8 mEq/L at 8 hours, and 2% had increases >12 mEq/L at 24 hours. 2
  • Initiate and re-initiate tolvaptan only in a hospital setting where serum sodium can be monitored closely. 2
  • Avoid fluid restriction during the first 24 hours of therapy, as it increases the risk of overly rapid sodium correction. 2

Hepatotoxicity Monitoring

  • Monthly liver function tests are mandatory for the first 18 months, then every 3 months until discontinuation in ADPKD patients. 1, 6
  • Obtain morning blood samples before the tolvaptan dose. 1
  • Limit tolvaptan duration to 30 days in hyponatremia patients to minimize liver injury risk. 2

Fluid and Electrolyte Management

  • Patients should drink 2–3 L of water daily, avoiding sugary or fatty beverages, and follow a low-sodium diet to mitigate polyuria. 6
  • Monitor serum sodium regularly to assess adequacy of water intake and prevent over-correction. 1, 6
  • Thirst (16% of patients) and dry mouth (8%) are common aquaretic effects that may limit tolerability. 6

Dosing Considerations for ADPKD

  • Initiate with 45 mg in the morning and 15 mg eight hours later; uptitrate weekly to a target of 90 mg morning and 30 mg afternoon. 1, 6
  • After at least one week, increase to 60 mg morning and 30 mg afternoon, then after another week to the target maintenance dose. 6
  • Continue treatment until approaching kidney replacement therapy if well tolerated. 1, 6

Common Pitfalls to Avoid

  • Do not use tolvaptan in hypovolemic hyponatremia—it will worsen volume depletion. 2, 5
  • Do not combine with strong CYP3A inhibitors—this is an absolute contraindication. 2
  • Do not restrict fluids during the first 24 hours of hyponatremia treatment—this increases the risk of overly rapid sodium correction. 2
  • Do not continue tolvaptan during intercurrent illnesses causing volume depletion—patients must skip doses during "sick days." 1, 6
  • Do not overlook monthly liver function monitoring in ADPKD patients—hepatotoxicity typically occurs within the first 18 months. 1, 6
