Vimovo Dosing

The recommended adult dose of Vimovo is one tablet twice daily, with either 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole, taken at least 30 minutes before meals. 1

Administration Instructions

  • Swallow tablets whole with liquid - do not split, chew, crush, or dissolve 1
  • Take at least 30 minutes before meals to ensure proper absorption 1
  • Antacids may be used while taking Vimovo 1
  • If a dose is missed, take it as soon as possible unless the next scheduled dose is due; never double up on doses 1

Dosing by Patient Population

Adults with Rheumatoid Arthritis, Osteoarthritis, or Ankylosing Spondylitis

  • One tablet twice daily of either:
    • 375 mg naproxen/20 mg esomeprazole, OR
    • 500 mg naproxen/20 mg esomeprazole 1

Adolescents (12 years and older) with Juvenile Idiopathic Arthritis

Weight-based dosing is critical:

  • 38 kg to less than 50 kg: One 375 mg naproxen/20 mg esomeprazole tablet twice daily 1
  • Greater than 50 kg: One tablet twice daily of either 375 mg naproxen/20 mg esomeprazole OR 500 mg naproxen/20 mg esomeprazole 1

Special Populations

Renal Impairment

  • Avoid in moderate to severe renal impairment (creatinine clearance <30 mL/min) 1
  • Naproxen-containing products are not recommended in this population due to increased risk of adverse effects 1

Hepatic Impairment

  • Mild to moderate hepatic impairment: Monitor closely and consider dose reduction based on the naproxen component 1
  • Severe hepatic impairment: Avoid Vimovo entirely 1

Important Dosing Considerations

Use the lowest effective naproxen dose for the shortest duration consistent with individual patient treatment goals to minimize cardiovascular and gastrointestinal risks 1

Do not substitute Vimovo with separate naproxen and esomeprazole products - the fixed-dose combination is specifically formulated with enteric-coated naproxen and immediate-release esomeprazole 1

Vimovo is not recommended for initial treatment of acute pain because naproxen absorption is delayed compared to other naproxen-containing products 1

If less than 40 mg total daily esomeprazole is appropriate, consider a different treatment - Vimovo does not allow for lower esomeprazole dosing 1

Duration of Therapy

Controlled studies do not extend beyond 6 months, so long-term use beyond this timeframe lacks robust safety and efficacy data 1
