Maximum Daily Dose of Ursodeoxycholic Acid (Ursodiol)
The maximum safe daily dose of ursodeoxycholic acid is 20 mg/kg/day, and doses should not exceed this threshold. Higher doses of 28-30 mg/kg/day have been associated with significantly worse clinical outcomes, including increased risk of liver transplantation and development of varices, despite biochemical improvement 1.
Dose Recommendations by Indication
Primary Biliary Cholangitis (PBC)
- Standard therapeutic dose: 13-15 mg/kg/day 2, 3
- This dose is recommended as first-line therapy for all patients with PBC 2
- Doses of 13-15 mg/kg/day have been shown to be optimal, with no additional benefit from higher doses 4
- The FDA-approved dosing for gallstone dissolution is 8-10 mg/kg/day given in 2-3 divided doses 3
Primary Sclerosing Cholangitis (PSC)
- Recommended dose: 15-20 mg/kg/day 5
- Critical safety warning: Do NOT exceed 20 mg/kg/day 1
- A multicenter study using high doses of 28-30 mg/kg/day in PSC patients was terminated early due to enhanced risk of reaching primary endpoints (liver transplantation, development of varices) in the UDCA treatment group 6
- European consensus guidelines explicitly state: "ursodiol doses should not exceed 20 mg/kg/day" 1
Pregnancy (PBC/PSC/Intrahepatic Cholestasis)
- Recommended dose: 10-20 mg/kg/day 5
- Start with 10 mg/kg/day and slowly increase up to 20 mg/kg/day as needed 5
- UDCA should be continued during pregnancy in PBC and PSC as it is safe during pregnancy and breastfeeding 5
Gallstone Prevention (Rapid Weight Loss)
- Recommended dose: 600 mg/day (300 mg twice daily) 3
- This is specifically for patients undergoing rapid weight loss after bariatric surgery 3
Important Clinical Caveats
Dose-Response Relationship
- Studies using 20-25 mg/kg/day showed improvements in histological grade of liver fibrosis and cholangiographic appearances in PSC 6
- However, the risk-benefit ratio becomes unfavorable above 20 mg/kg/day 1
- Doses of 13-15 mg/kg/day appear optimal for PBC, with no additional benefit from higher doses 4
Safety Considerations
- High-dose UDCA (28-30 mg/kg/day) may be harmful in late-stage PSC 6
- The aborted multicenter trial demonstrated that despite biochemical improvement, patients on 28-30 mg/kg/day had worse clinical outcomes 6
- UDCA is generally well-tolerated with minimal side effects at recommended doses 3
