Tobramycin Reconstitution and Dilution for Intravenous Administration
For intravenous administration, reconstitute the 1.2 g tobramycin pharmacy bulk vial with 30 mL of Sterile Water for Injection to achieve a 40 mg/mL solution, then further dilute the required dose in 50-100 mL of 0.9% Sodium Chloride or 5% Dextrose for adults (proportionately less for pediatric patients) and infuse over 20-60 minutes. 1
Reconstitution Process
Step 1: Initial Reconstitution
- Add 30 mL of Sterile Water for Injection, USP to the 1.2 g tobramycin pharmacy bulk vial 1
- This produces a 40 mg/mL concentration 1
- Use aseptic technique throughout the preparation 1
Step 2: Further Dilution for IV Infusion
- Withdraw the calculated dose volume from the reconstituted 40 mg/mL solution 1
- Add this to 50-100 mL of diluent for adult doses (either 0.9% Sodium Chloride Injection or 5% Dextrose Injection) 1
- For pediatric patients, use proportionately less diluent than for adults 1
Administration Guidelines
Infusion Rate
- Infuse the diluted solution over 20-60 minutes 1
- Do not infuse in less than 20 minutes as peak serum concentrations may exceed 12 mcg/mL, increasing toxicity risk 1
Timing Constraints
- After penetrating the pharmacy bulk vial, dispense entire contents within 24 hours 1
- Visually inspect for particulate matter and discoloration before administration 1
Important Safety Considerations
Peak Concentration Management
- The 20-60 minute infusion window is critical to avoid peak concentrations above 12 mcg/mL, which are associated with increased nephrotoxicity and ototoxicity 1
- Rapid IV administration (less than 20 minutes) can produce transient peaks exceeding 20 mcg/mL in some patients 2
Drug Incompatibility
- Never physically premix tobramycin with other drugs 1
- Administer separately according to recommended dose and route 1
Monitoring Requirements
- Measure peak serum concentrations approximately 30 minutes after completing the IV infusion 1
- Target peak concentrations of 4-6 mcg/mL for adequate therapy 2
- Avoid peak concentrations above 12 mcg/mL 1
- Monitor trough concentrations (drawn just before next dose) and keep below 2 mcg/mL to prevent accumulation 1
