Antithrombotic Therapy Duration After TAVR with Bioprosthetic Valve
For patients without an indication for oral anticoagulation, aspirin 75-100 mg daily alone is the recommended therapy after TAVR, and this should be continued indefinitely. 1
Primary Recommendation for Patients WITHOUT Atrial Fibrillation or Other Anticoagulation Indications
Single antiplatelet therapy (SAPT) with aspirin 75-100 mg daily is reasonable and should be the standard approach for patients after bioprosthetic TAVR who do not have other indications for anticoagulation. 1
Dual antiplatelet therapy (DAPT) with aspirin plus clopidogrel 75 mg may be considered for 3-6 months in patients at low bleeding risk, but this is a weaker recommendation (Class 2b). 1
Recent high-quality evidence demonstrates that SAPT reduces major bleeding risk compared to DAPT without increasing thrombotic events, making it the preferred strategy. 2
The duration of aspirin therapy should be indefinite in the absence of bleeding complications or other contraindications. 1
Alternative Regimens (Weaker Evidence)
Vitamin K Antagonist (VKA) Option
Anticoagulation with warfarin to achieve an INR of 2.5 may be reasonable for at least 3 months after TAVR in patients at low bleeding risk, though this is also a Class 2b recommendation. 1
One single-center study showed that 3-month warfarin therapy was well-tolerated with similar or lower bleeding rates compared to antiplatelet therapy, though clinical outcomes were not significantly different. 3
What NOT to Use
- Low-dose rivaroxaban (10 mg daily) plus aspirin is contraindicated after bioprosthetic TAVR in patients without other indications for anticoagulation, as this combination significantly increases bleeding risk without reducing thrombotic events. 1
For Patients WITH Atrial Fibrillation or Other Anticoagulation Indications
Oral anticoagulation alone (either VKA or DOAC) should be used without adding antiplatelet therapy in most cases, as the combination increases bleeding risk substantially. 1, 4
If antiplatelet therapy must be added (e.g., recent PCI), this should be done with extreme caution and for the shortest duration possible due to markedly increased bleeding risk. 1
Whether DOACs are superior to VKAs in this population remains uncertain, as comparative data are limited. 4
Key Clinical Considerations
Clopidogrel vs. Aspirin for SAPT
- If choosing SAPT, clopidogrel 75 mg daily may be preferable to aspirin, as one registry study showed lower cardiovascular mortality with clopidogrel compared to aspirin at 2 years post-TAVR, regardless of anticoagulation use. 5
Subclinical Leaflet Thrombosis
While anticoagulation can prevent and resolve subclinical leaflet thrombosis (which occurs in up to 13% of TAVR patients), the clinical significance of this finding remains unclear, and the bleeding risk associated with routine anticoagulation is unacceptably high. 6, 4
The relationship between subclinical leaflet thrombosis and structural valve deterioration requires further study. 7
Bleeding Risk Assessment
The bleeding risk with any antithrombotic strategy must be carefully weighed, as major bleeding events are strongly associated with mortality after TAVR. 8
Network meta-analysis confirms that SAPT provides the best balance, ranking highest for reducing bleeding while maintaining protection against thrombotic events. 2
