Lomotil and Racecadotril: Clinical Guide
Lomotil (Diphenoxylate/Atropine)
Indications
Lomotil is indicated as adjunctive therapy for diarrhea management in patients 13 years of age and older 1. It reduces stool frequency and volume in chronic diarrhea, decreasing average stool frequency from 4.9 to 2.6 times/day and stool weight from 460 to 256 g/day 2.
Contraindications
Lomotil is absolutely contraindicated in 1:
- Pediatric patients less than 6 years of age due to risks of respiratory and CNS depression
- Diarrhea associated with C. difficile or other enterotoxin-producing bacteria due to risk of GI complications including sepsis
- Known hypersensitivity to diphenoxylate or atropine
- Obstructive jaundice
Critical caveat: Avoid Lomotil in bloody diarrhea, fever with diarrhea, or suspected inflammatory diarrhea at any age due to risk of toxic megacolon 3. The IDSA strongly recommends against antimotility agents in these scenarios 3.
Dosage and Duration
While the FDA label indicates use for patients ≥13 years 1, specific dosing details are not provided in the evidence. Based on standard practice and the evidence showing therapeutic effects 2, treatment should be:
- Used intermittently or temporarily rather than continuously
- Discontinued once diarrhea resolves (typically 24-48 hours) 4
- Not used prophylactically or at fixed high doses 4
Administration Instructions
- Take as adjunctive therapy alongside oral rehydration 1
- Monitor for signs of ileus, abdominal distension, or lethargy 3
- Stop immediately if fever develops or bloody stools appear 3
Racecadotril
Indications
Racecadotril is recommended as adjunctive antisecretory therapy for acute watery diarrhea in both adults and children 3, 5, 6. It works by inhibiting enkephalinase, reducing intestinal water and electrolyte secretion without affecting motility 7, 8.
Contraindications
The evidence does not specify absolute contraindications for racecadotril. However:
- Should be avoided in bloody diarrhea or inflammatory conditions (same precautions as other antidiarrheals) 3
- Not available in North America 3
Dosage and Duration
- 100 mg three times daily orally
- Continue until diarrhea resolves (mean duration approximately 55 hours or ~2.3 days)
- Use as add-on to oral rehydration solution
- Age-appropriate dosing as adjunct to ORS
- Continue for 3-5 days or until symptom resolution
- Reduces time to cure by approximately 28 hours compared to standard treatment 5
Administration Instructions
- Always combine with oral rehydration solution 8
- Take three times daily with meals 7
- Monitor for symptom resolution; most patients improve within 3 days 5
Comparative Efficacy
Racecadotril demonstrates superior tolerability compared to Lomotil/loperamide 7, 6:
- Similar efficacy in resolving diarrhea (55 hours for both racecadotril and loperamide) 7
- Significantly less rebound constipation: 16% vs 25% with loperamide 7
- Better abdominal symptom relief: faster resolution of abdominal pain and distension 7
- Fewer adverse events: 14.2% vs 23.9% with loperamide 7
- Tolerability comparable to placebo in children 5
Key clinical advantage: Racecadotril reduces stool volume without affecting intestinal motility, avoiding the complications associated with antimotility agents 8. This makes it safer in situations where the etiology of diarrhea is uncertain 6.
Critical Safety Warnings
- Never use in children under 6 years - risk of fatal respiratory depression
- One case report documented severe toxicity in an infant given adult doses, causing respiratory depression and miosis, requiring naloxone reversal 9
- Deaths reported in 0.54% of children given loperamide (similar agent), all under 3 years 3
