Paxlovid and Zetia Interaction
Paxlovid (nirmatrelvir/ritonavir) can be safely co-administered with Zetia (ezetimibe) without dose adjustment or significant clinical concern.
Mechanism and Safety Profile
The ritonavir component of Paxlovid is a strong CYP3A4 inhibitor that causes numerous drug-drug interactions through this metabolic pathway 1. However, ezetimibe does not undergo significant metabolism via CYP3A4 and is therefore not subject to the primary mechanism by which Paxlovid causes problematic drug interactions 2.
Key Pharmacokinetic Considerations
Ezetimibe metabolism: Ezetimibe is primarily metabolized through glucuronidation in the intestine and liver, not through the CYP3A4 system that ritonavir inhibits 2.
Transporter interactions: While ritonavir and nirmatrelvir can inhibit P-glycoprotein (P-gp) and other transporters (OATP1B1/B3, BCRP), ezetimibe is not significantly affected by these transporter inhibitions in a clinically meaningful way 2.
No contraindication listed: The FDA-approved Paxlovid prescribing information does not list ezetimibe among contraindicated or cautionary medications 3.
Contrast with Other Lipid-Lowering Agents
This safety profile stands in stark contrast to certain statins, which are contraindicated with Paxlovid:
Lovastatin and simvastatin are absolutely contraindicated due to ritonavir's strong CYP3A4 inhibition causing markedly elevated plasma concentrations and risk of serious adverse events including rhabdomyolysis 2, 3.
Atorvastatin is also contraindicated with ritonavir-containing regimens 2.
Rosuvastatin use is not recommended with ledipasvir/sofosbuvir due to OATP inhibition, though this specific concern does not apply to Paxlovid 2.
Clinical Management
No dose adjustment or treatment interruption of ezetimibe is required when initiating Paxlovid 2. This makes ezetimibe a particularly valuable option for lipid management in patients requiring Paxlovid therapy who cannot safely continue their statin medications 4.
Practical Approach
Continue ezetimibe at standard dosing (10 mg daily) throughout the 5-day Paxlovid course 3.
No additional monitoring beyond standard care is necessary for this specific drug combination 2.
If the patient is on a contraindicated statin (lovastatin, simvastatin, atorvastatin), temporarily discontinue the statin during Paxlovid treatment but continue ezetimibe to maintain some lipid-lowering therapy 2, 3.
Important Caveat
While ezetimibe itself does not interact with Paxlovid, always review the patient's complete medication list for other potentially interacting drugs, as ritonavir causes extensive drug-drug interactions with numerous medication classes including anticoagulants, calcium channel blockers, immunosuppressants, antiarrhythmics, and many others 1, 5, 6, 7, 8.
