Tetanus Toxoid Dosing for Children
The standard dose of tetanus toxoid injection for children is 0.5 mL administered intramuscularly, regardless of age, as part of combination vaccines (DTaP for children under 7 years, Td/Tdap for children 7 years and older). 1
Primary Immunization Series
Children Under 7 Years of Age
- DTaP vaccine (containing tetanus toxoid with diphtheria and pertussis) is the preferred formulation 2
- Dose volume: 0.5 mL intramuscularly 1
- Primary series: 3-4 doses starting at 6 weeks of age, with doses typically given at 2,4,6 months, and a fourth dose at 15-18 months 2
- Final dose: Administered at age 4-6 years, provided 6 months have elapsed since the previous dose 2
Children 7 Years and Older
- Td vaccine (reduced diphtheria content) is used instead of DTaP 1
- Dose volume: 0.5 mL intramuscularly 1
- Primary series for unvaccinated children: 3 doses total—first 2 doses given 2 months apart, third dose given 6-8 months after the second dose 1
Booster Immunization
- Tdap booster: Single dose at age 11-12 years for those who completed the childhood DTaP series 2
- Subsequent Td boosters: Every 10 years thereafter 1
- Dose volume: 0.5 mL intramuscularly for all booster doses 1
Administration Details
- Route: Intramuscular injection only—never intravenous or subcutaneous 1
- Site: Deltoid muscle is preferred; avoid gluteal area or sites near major nerve trunks 1
- Preparation: Shake vial/syringe until uniform white cloudy suspension forms 1
Wound Management Considerations
For children 7 years and older with tetanus-prone wounds:
- Clean, minor wounds: Td booster if >10 years since last dose 1
- All other wounds (contaminated, puncture, traumatic): Td booster if >5 years since last dose 1
- Unknown/incomplete vaccination: Both Td vaccine (0.5 mL) and Tetanus Immune Globulin (TIG) at separate sites 1
Critical Pitfalls to Avoid
- Do not reduce the dose volume: Always use 0.5 mL regardless of child's age or size—dose reduction compromises immunogenicity 1
- Avoid excessive boosting: More frequent dosing than recommended increases adverse reactions without improving protection 1
- Do not give boosters more frequently than every 5 years for wound management or every 10 years for routine boosting, as this can cause Arthus-type hypersensitivity reactions 1
