Contraindications to Trazodone Use
Trazodone is absolutely contraindicated in patients taking or within 14 days of stopping monoamine oxidase inhibitors (MAOIs), including linezolid or intravenous methylene blue, due to the risk of life-threatening serotonin syndrome. 1
Absolute Contraindications
- Concurrent MAOI use or within 14 days of MAOI discontinuation – This includes traditional MAOIs (phenelzine, tranylcypromine, isocarboxazid, selegiline) and MAOI-like drugs such as linezolid or intravenous methylene blue 1
- The 14-day washout period is mandatory before initiating trazodone after MAOI discontinuation 1
Strong Relative Contraindications (Avoid Use)
Cardiac conditions requiring avoidance:
- Preexisting cardiac arrhythmias – Trazodone is arrhythmogenic and can precipitate isolated PVCs, ventricular couplets, ventricular tachycardia with syncope, and torsade de pointes 1
- Known QT prolongation – Trazodone prolongs the QT/QTc interval; avoid in patients with congenital long QT syndrome 1
- Acute myocardial infarction recovery phase – Not recommended during initial MI recovery 1
- Symptomatic bradycardia, hypokalemia, or hypomagnesemia – These conditions increase torsade de pointes risk 1
Drug interaction scenarios requiring avoidance:
- Concurrent use with strong CYP3A4 inhibitors (itraconazole, ketoconazole, clarithromycin, voriconazole) – This combination increases trazodone exposure and cardiac arrhythmia risk 1
- Concurrent use with other QT-prolonging drugs – Including Class 1A antiarrhythmics (quinidine, procainamide), Class 3 antiarrhythmics (amiodarone, sotalol), certain antipsychotics (ziprasidone, chlorpromazine, thioridazine), and certain antibiotics (gatifloxacin) 1
Clinical Situations Where Trazodone Should Not Be Used
Primary chronic insomnia without depression:
- The American Academy of Sleep Medicine (2017) explicitly recommends against using trazodone for sleep onset or sleep maintenance insomnia 2
- The VA/DoD guideline (2020) strongly advises against trazodone for chronic insomnia disorder because low-quality evidence supporting efficacy is outweighed by its adverse effect profile 3
- Trazodone showed no differences in objective sleep efficiency or discontinuation rates compared to placebo, with only subjective sleep quality improvements 3
High-Risk Populations Requiring Extreme Caution
Elderly patients:
- Increased risk of orthostatic hypotension, falls, and cognitive impairment 1
- Higher incidence of cardiovascular adverse effects 4
Patients with cardiovascular disease:
- Close cardiac monitoring is mandatory if trazodone must be used 1
- Orthostatic hypotension risk is particularly elevated 5
Pediatric and young adult patients (under age 25):
- Increased risk of suicidal thoughts and behaviors – 14 additional cases per 1000 patients treated in those under 18 years, and 5 additional cases per 1000 in ages 18-24 1
Important Drug Interaction Contraindications
Serotonergic drug combinations requiring careful avoidance:
- Concurrent use with other serotonergic drugs (SSRIs, SNRIs, triptans, tricyclic antidepressants, fentanyl, lithium, tramadol, tryptophan, buspirone, St. John's Wort) significantly increases serotonin syndrome risk 1
- If combination is unavoidable, intensive monitoring for serotonin syndrome symptoms (mental status changes, autonomic instability, neuromuscular symptoms, seizures) is mandatory 1
Anticoagulant/antiplatelet combinations:
- Concurrent use with warfarin, rivaroxaban, dabigatran, or clopidogrel potentiates bleeding risk; requires careful INR monitoring with warfarin 1
Common Pitfalls to Avoid
- Do not prescribe trazodone as first-line therapy for primary insomnia – Despite widespread off-label use, guideline evidence does not support this practice 3, 2, 6, 7
- Do not assume trazodone is "safer" than other antidepressants – It carries significant cardiac risks including arrhythmias and orthostatic hypotension 1, 8
- Do not overlook the 14-day MAOI washout period – This is an absolute requirement to prevent fatal serotonin syndrome 1
- Do not use in patients requiring strong CYP3A4 inhibitors – The combination increases cardiac arrhythmia risk substantially 1
