No Waiting Period Required When Switching from Ezetimibe to Atorvastatin
You can start atorvastatin immediately after discontinuing ezetimibe without any required washout period. There is no pharmacological interaction or safety concern that necessitates a waiting interval between these two medications.
Rationale
Pharmacokinetic Considerations
Ezetimibe has a half-life of approximately 22 hours for both the parent compound and its active glucuronide metabolite 1. However, this does not create any safety concern for immediate transition to atorvastatin because:
- Different mechanisms of action: Ezetimibe inhibits cholesterol absorption in the small intestine via the NPC1L1 transporter 1, while atorvastatin inhibits HMG-CoA reductase in the liver
- No drug-drug interactions: Ezetimibe does not inhibit or induce cytochrome P450 enzymes (1A2, 2D6, 2C8/9, or 3A4) 1, which are involved in statin metabolism
- Complementary pathways: These agents work through entirely separate pathways with no overlapping toxicity concerns
Clinical Practice Guidelines
The 2022 ACC Expert Consensus Decision Pathway discusses adding ezetimibe to statins or using them in combination, but nowhere in the guidelines is there any mention of required washout periods when switching between lipid-lowering agents 2. The FDA label for ezetimibe similarly contains no warnings about timing when discontinuing the medication 1.
Practical Implementation
- Discontinue ezetimibe on any given day
- Start atorvastatin the same day or the next day based on convenience
- Reassess LDL-C as early as 4 weeks after initiating atorvastatin 1
- Monitor for typical statin-related adverse effects (myopathy, hepatic transaminase elevations)
Important Caveat
The only timing consideration with ezetimibe relates to bile acid sequestrants (if the patient were taking one): ezetimibe should be administered either ≥2 hours before or ≥4 hours after a bile acid sequestrant 1. This does not apply to switching to atorvastatin.
