For COPD exacerbation, azithromycin is typically prescribed for a duration of 1 year, with a regimen of 250 mg or 500 mg daily three times a week, as recommended by GOLD2020 2. This long-term administration of low-dose azithromycin has been shown to suppress proinflammatory cytokine production, potentiate macrophage phagocytosis, and anti-inflammatory cytokine expression, ultimately reducing the frequency of exacerbations in patients with COPD. The beneficial effects of azithromycin on reducing the number of exacerbations in patients with COPD have been demonstrated in several clinical studies, including the COLUMBUS study, which found that azithromycin was most effective in patients with COPD with frequent exacerbations in GOLD 1 and 2 and GOLD C, as well as in patients with blood eosinophilia over 2% 2. A study published in the New England Journal of Medicine in 2011 also found that azithromycin taken daily for 1 year decreased the frequency of exacerbations and improved quality of life in patients with COPD, although it caused hearing decrements in a small percentage of subjects 3. Other studies have also investigated the use of azithromycin in COPD exacerbation, including a Cochrane review that compared the safety and efficacy of different classes of antibiotics for prophylaxis of exacerbations in patients with COPD, but the evidence was found to be of very low certainty due to imprecision and methodological quality of the included studies 4. In clinical practice, the treatment duration of azithromycin for COPD exacerbation may vary depending on the severity of the exacerbation and the patient's underlying condition, but the current evidence suggests that a 1-year course of azithromycin is effective in reducing the frequency of exacerbations and improving quality of life in patients with COPD. Some key points to consider when prescribing azithromycin for COPD exacerbation include:
- The medication should be taken with or without food, but not with antacids containing aluminum or magnesium
- Patients should be monitored for potential side effects, including gastrointestinal disturbances, QT interval prolongation, and liver enzyme elevations
- Caution is needed in patients with cardiac or hepatic conditions
