Prevnar 20 (PCV20): Indications, Dosing, and Contraindications

Prevnar 20 is recommended as a single dose for all adults ≥50 years of age who have not previously received a pneumococcal conjugate vaccine, and as a 4-dose series (at 2,4,6, and 12-15 months) for children starting at 6 weeks of age. 1

Adult Indications and Dosing

Age-Based Recommendations

All adults ≥50 years: A single 0.5 mL intramuscular dose of PCV20 is recommended for pneumococcal vaccine-naïve adults 2. This represents an expansion from the previous ≥65 years recommendation, now including adults 50-64 years to reduce pneumococcal disease burden in this population 3.

Adults 19-49 years with risk conditions: PCV20 is indicated for adults with:

  • Immunocompromising conditions: chronic renal failure, nephrotic syndrome, immunodeficiency, iatrogenic immunosuppression, HIV, malignancy, organ transplant, asplenia, sickle cell disease 2
  • Chronic medical conditions: alcoholism, chronic heart/liver/lung disease, diabetes, smoking 2
  • Anatomic conditions: CSF leak or cochlear implant 2

Previously Vaccinated Adults

The 2024 ACIP guidelines provide specific algorithms based on vaccination history 2:

For adults ≥65 years:

  • No prior vaccination or PCV7 only: Single dose of PCV20 (or PCV21/PCV15 alternatives)
  • PPSV23 only: PCV20 ≥1 year after last PPSV23
  • PCV13 only: PCV20 ≥1 year after PCV13
  • PCV13 + PPSV23 (both before age 65): PCV20 ≥5 years after last pneumococcal vaccine
  • PCV13 + PPSV23 (PPSV23 at ≥65 years): Shared clinical decision-making for PCV20 ≥5 years after last dose

For adults 19-64 years with immunocompromising conditions/CSF leak/cochlear implant:

  • No prior vaccination: Single dose PCV20
  • PPSV23 only: PCV20 ≥1 year after PPSV23
  • PCV13 only: PCV20 ≥1 year after PCV13
  • PCV13 + PPSV23: PCV20 ≥5 years after last pneumococcal vaccine (completes series—no additional doses needed) 2

Critical timing note: When PCV20 is used after prior pneumococcal vaccination, it completes the series and no additional pneumococcal vaccines are needed 2.

Pediatric Indications and Dosing

Routine Infant Series

4-dose series at 2,4,6, and 12-15 months of age (first dose as early as 6 weeks) 1. This prevents invasive pneumococcal disease from all 20 serotypes and otitis media from 7 serotypes (4, 6B, 9V, 14, 18C, 19F, 23F) 1.

Catch-Up Schedules for Unvaccinated Children

Ages 7-11 months: 3 doses (first 2 doses ≥4 weeks apart; third dose after first birthday, ≥2 months after second dose)

Ages 12-23 months: 2 doses ≥2 months apart

Ages ≥24 months through 17 years: Single dose 1

Previously Vaccinated with Lower-Valency PCV

Ages 15 months-17 years: Single dose of PCV20 ≥8 weeks after last lower-valency PCV dose 1

Administration

  • Route: Intramuscular injection only
  • Preparation: Shake vigorously until homogeneous white suspension; inspect for particulate matter
  • Do not mix with other vaccines in same syringe
  • Can be coadministered with other age-appropriate vaccines at different injection sites 2, 1

Contraindications

Absolute contraindications 2, 1:

  • Severe allergic reaction (anaphylaxis) to any PCV20 component
  • Severe allergic reaction to diphtheria toxoid or any diphtheria toxoid-containing vaccine

Important Precautions

Altered immunocompetence: While PCV20 is indicated for immunocompromised individuals, immune responses may be reduced. Consider individual benefit-risk assessment 1.

Premature infants: Apnea has been observed following intramuscular vaccination in premature infants. Weigh individual medical status against benefits and risks when timing vaccination 1.

Immunosuppressive therapy timing: Administer PCV20 before starting immunosuppressive therapy or at diagnosis when possible, as immune responses are significantly impaired during treatment 4.

Post-HSCT patients: Revaccination is essential post-transplant as hematopoietic stem cell transplantation eliminates immunologic memory 4.

Special Consideration: Serotype 4 Coverage

Critical caveat: In populations where ≥30% of pneumococcal disease is serotype 4, PCV20 alone or PCV15+PPSV23 are preferred over PCV21 because PCV21 lacks serotype 4 coverage 2. This is a key clinical decision point when selecting among available PCVs.

Common Adverse Reactions

Infants/children (4-dose series): Irritability (>60%), injection site pain (>30%), drowsiness (>30%), decreased appetite and injection site redness (>20%) 1

Adults 18-59 years: Injection site pain (>70%), muscle pain (>50%), fatigue (>40%), headache (>30%) 1

Adults ≥60 years: Injection site pain (>50%), muscle pain and fatigue (>30%), headache (>20%) 1

Most reactions are mild-to-moderate and transient 5, 6, 7.
