PCV Dose and Route of Administration
The pneumococcal conjugate vaccine (PCV13 or PCV20) is administered as a single 0.5 mL dose via intramuscular (IM) injection 1, 2.
Route of Administration
- Intramuscular injection only - PCV must be given IM, not subcutaneously or intravenously 1, 2
- The vaccine is available in latex-free, single-dose, prefilled syringes containing 0.5 mL 1
- Can be administered concurrently with other routine vaccines if given in a separate syringe at a separate injection site 1
Dosing by Age and Vaccination History
Infants and Young Children (Previously Unvaccinated)
Infants 2-6 months:
- 3-dose primary series at 2,4, and 6 months (minimum 4-week intervals)
- Fourth booster dose at 12-15 months (minimum 8 weeks after third dose) 1
Infants 7-11 months:
- 2 doses with minimum 4-week interval
- Third dose at 12-15 months (minimum 8 weeks after second dose) 1
Children 12-23 months:
- 2 doses with minimum 8-week interval 1
Healthy children 24-59 months:
- Single 0.5 mL dose 1
Children 24-71 months with chronic diseases or immunocompromising conditions:
- 2 doses with minimum 8-week interval 1
Adults
Adults ≥50 years (PCV-naïve):
- Single 0.5 mL dose of PCV20 or PCV21 3
Adults with immunocompromising conditions, CSF leak, or cochlear implant:
Critical Administration Points
- Minimum age for first dose: 6 weeks 1
- Premature infants: Administer at chronologic age (not corrected age) if medically stable 1
- Do not administer PCV13 and PPSV23 concurrently - safety and efficacy of concurrent administration has not been studied 1
- When both vaccines are indicated, PCV should be given first, followed by PPSV23 at least 8 weeks later 5
Common Pitfall
The most frequent error is confusing the 0.5 mL dose volume with dosing schedules. Every PCV dose is 0.5 mL regardless of age or indication - what changes is the number of doses and timing between doses, not the volume per dose 1, 2.
