Dantrolene Dosage
For acute malignant hyperthermia (MH) crisis, administer intravenous dantrolene at an initial dose of 2-3 mg/kg actual body weight, with additional 1 mg/kg boluses every 5 minutes until treatment goals are achieved (ETCO2 <6 kPa, core temperature <38.5°C, normal minute ventilation), continuing up to 10 mg/kg or beyond if MH persists. 1, 2
Intravenous Dantrolene for Malignant Hyperthermia
Acute MH Crisis Treatment
The 2021 Association of Anaesthetists guideline provides the most current evidence-based approach 1:
- Initial dose: 2-3 mg/kg IV push (based on actual body weight, not ideal body weight)
- Administration technique: Give each reconstituted syringe immediately as prepared rather than waiting for the complete dose—this is critical given reconstitution takes up to 5 minutes per vial
- Additional boluses: 1 mg/kg every 5 minutes until treatment goals achieved
- Treatment endpoints:
- ETCO2 <6 kPa with normal minute ventilation
- Core temperature <38.5°C
- Resolution of metabolic abnormalities
Critical dosing consideration: When cumulative dose reaches 10 mg/kg, formally reassess the diagnosis. If MH remains likely, continue dantrolene beyond the 10 mg/kg maximum stated in product labeling—this is a life-saving exception where guideline recommendations supersede package insert limits 1. The FDA label states a maximum of 10 mg/kg 2, but the most recent guideline evidence supports exceeding this when clinically indicated.
Pediatric Dosing
Pharmacokinetic studies demonstrate that children require the same weight-based dosing as adults 1:
- Initial: 2-3 mg/kg IV
- Additional boluses: 1 mg/kg every 5 minutes
- Dose based on actual body weight
Obese Patients
Use actual body weight for all calculations, not ideal body weight. While no pharmacokinetic studies exist in obese patients, lipophilic drugs like dantrolene require loading doses based on actual weight 1.
MH Recrudescence Management
Recrudescence can occur up to 14 hours after initial control 1:
- If recurrence within 6 hours: Give 1 mg/kg initially
- If recurrence >6 hours after last dose: Give 2-3 mg/kg (restart as if new crisis)
- Do NOT use prophylactic dantrolene after controlling initial reaction—it causes unnecessary muscle weakness and nausea without proven benefit 1
Important caveat: Avoid continuous infusion due to high incidence of thrombophlebitis from solution osmolarity 1.
Prophylactic Dosing (Preoperative)
For known MH-susceptible patients undergoing surgery 2:
IV prophylaxis: 2.5 mg/kg infused over 1 hour, starting 1.25 hours before anesthesia
Oral prophylaxis (alternative): 4-8 mg/kg/day divided into 3-4 doses for 1-2 days before surgery, with last dose 3-4 hours preoperatively with minimal water
The 2021 European MH Group guideline recommends avoiding prophylactic dantrolene entirely if trigger-free anesthesia can be reliably provided 3. This represents a shift from older practices.
Post-Crisis Follow-Up
After an MH crisis 2:
- Oral dantrolene: 4-8 mg/kg/day in 4 divided doses for 1-3 days
- IV alternative: 1 mg/kg or more as needed if oral administration impractical
Oral Dantrolene for Spasticity
For chronic spasticity (cerebral palsy, spinal cord injury, stroke, multiple sclerosis) 4:
Titration schedule:
- Start: 25 mg once daily
- Increase to: 25 mg 2-4 times daily
- Escalate by: 25 mg increments
- Maximum: Up to 100 mg 2-4 times daily (400 mg/day total)
- Use lowest effective dose
Important limitation: Muscle weakness is the principal dose-limiting side effect, particularly problematic in ambulatory patients with multiple sclerosis. Avoid in patients with pre-existing bulbar or respiratory weakness 4.
Special Populations
Liver Disease
Critical warning: Oral dantrolene carries risk of hepatotoxicity:
- Chemical liver dysfunction: 0.7-1% of patients on long-term therapy
- Symptomatic hepatitis: 0.35-0.5%
- Fatal hepatitis: 0.1-0.2% 4
Hepatitis can occur even at low doses (<100 mg/day) and as early as day 3 of therapy 5. However, this primarily applies to chronic oral use—IV dantrolene for acute MH treatment should never be withheld due to liver disease concerns, as untreated MH has 75-100% mortality with delayed treatment 6.
Elderly
No specific dose adjustments recommended. Use actual body weight for IV dosing. For oral therapy, start at lower end of dosing range and titrate carefully given increased risk of weakness and falls.
Reconstitution and Administration
- Traditional formulation: 20 mg vials reconstituted with 60 mL sterile water (requires vigorous shaking for up to 5 minutes) 1, 2
- Ryanodex (not available in UK/Europe): 250 mg vials dissolved in 5 mL water 1
- Incompatible solutions: Do NOT use 5% dextrose, 0.9% saline, or other acidic solutions 2
- Stability: Use within 6 hours of reconstitution; store at 15-30°C 2
- Protect from light after reconstitution
Common Pitfalls
- Waiting to accumulate full initial dose: Administer each syringe as soon as reconstituted—time is critical 1
- Stopping at 10 mg/kg: Continue beyond this if MH persists; mortality approaches 100% with treatment delays beyond 50 minutes 1, 7
- Using ideal body weight in obese patients: Always use actual body weight 1
- Prophylactic post-crisis dosing: Not recommended—causes unnecessary side effects 1, 3
- Continuous infusion: Causes thrombophlebitis; use intermittent boluses instead 1
Treatment Outcomes
Early dantrolene administration is life-saving. Historical data shows 75% mortality when treatment delayed >24 hours 6, with complications increasing every 10 minutes of delay, reaching 100% at 50-minute delay 7. With prompt dantrolene treatment, mortality has decreased to 1-10% 7, with most properly treated patients recovering without sequelae 6.
