Transitioning to Aripiprazole Long-Acting Injectable (LAI) from Oral Abilify 5 mg
Continue oral aripiprazole 5 mg daily for 14 consecutive days after administering the first aripiprazole once-monthly (AOM) 400 mg intramuscular injection, then discontinue oral therapy. This is the standard FDA-approved transition protocol that ensures therapeutic plasma concentrations are maintained during the LAI loading phase 1.
Transition Protocol
Step 1: Assess Current Stability
Your patient is on a relatively low dose (5 mg) of oral aripiprazole. Before transitioning:
- Confirm the patient is clinically stable on this dose
- Document current symptom control and tolerability
- Note that 5 mg is below the typical therapeutic range (10-30 mg/day for schizophrenia), so you may need to consider whether dose optimization is needed 1
Step 2: Initiate LAI with Oral Overlap
Day 1 of transition:
- Administer aripiprazole once-monthly (AOM) 400 mg intramuscularly (deltoid or gluteal)
- Continue oral aripiprazole 5 mg daily
- The oral supplementation period is 14 days minimum (some sources recommend up to 21 days) 1
Critical timing consideration: AOM takes approximately 4 months to reach steady-state plasma concentrations, which is why oral supplementation is essential during the initial phase 2.
Step 3: Discontinue Oral Therapy
- After 14 days of concurrent oral therapy, discontinue the oral aripiprazole
- Schedule the second AOM 400 mg injection for 4 weeks (28 days) after the first injection
Step 4: Maintenance Dosing
- Continue AOM 400 mg every 4 weeks (monthly)
- The 400 mg dose is appropriate for most patients, though doses can range from 300-400 mg based on tolerability 1, 2
Important Clinical Considerations
Dose equivalence caveat: Your patient is on 5 mg oral, which is lower than the typical starting dose. The AOM 400 mg formulation is designed for patients stabilized on oral aripiprazole 10-30 mg/day 1. Consider whether:
- The patient needs dose optimization before transitioning (increase oral dose first, then transition)
- The patient may benefit from the lower AOM 300 mg dose if 5 mg oral has been adequate
Alternative faster initiation option: If rapid therapeutic levels are needed, a 1-day initiation regimen using aripiprazole lauroxil nanocrystalline dispersion (ALNCD) plus a single 30 mg oral dose exists, but this is a different formulation than standard AOM 3.
Inpatient advantage: Research shows that initiating LAI during the first week of inpatient admission significantly reduces hospitalization time by an average of 12.1 days 4. Your inpatient setting is ideal for this transition.
Monitoring during transition:
- Assess for akathisia and extrapyramidal symptoms (LAI formulations show slightly higher rates than oral) 5, 2
- Monitor injection site reactions (typically mild to moderate pain)
- Evaluate clinical response at each monthly injection visit
- No post-injection observation period is required for AOM 2
If the patient misses doses: Reinitiation with oral overlap is only necessary if more than 6 weeks have elapsed since the last injection 1.
Alternative: 2-Month Formulation
If available and preferred, aripiprazole 2-month ready-to-use (Ari 2MRTU) 960 mg can be administered every 2 months after initial stabilization, though this still requires the same oral overlap period initially 6, 7, 8. This formulation is gluteal-only and offers extended dosing intervals for improved convenience.
Key advantage of LAI in this population: Long-acting injectables demonstrate superior adherence compared to oral medications and are particularly beneficial for patients with histories of non-adherence or frequent relapses 9, 10, 11. The inpatient setting provides an optimal opportunity to initiate this treatment and establish the injection routine before discharge.
