Abilify (Aripiprazole) Dosing for Adults
For adults with schizophrenia, start Abilify at 10-15 mg once daily; for bipolar disorder, start at 15 mg daily as monotherapy or 10-15 mg as adjunctive therapy; and for major depressive disorder as adjunctive treatment, start at 2-5 mg daily. 1
Schizophrenia Dosing
The recommended starting and target dose is 10 or 15 mg once daily, taken without regard to meals. 1 The FDA-approved dose range is 10-30 mg/day, though doses higher than 10-15 mg/day have not demonstrated superior efficacy 1.
Key Dosing Principles:
- Do not increase the dose before 2 weeks - this is the time required to reach steady-state plasma concentrations 1
- Full therapeutic effect may take 1-4 weeks to manifest 2
- Optimal efficacy occurs at 10 mg/day based on dose-response studies, with the highest response rates at this dose 3
- Doses above 20 mg/day provide no additional benefit and may paradoxically result in smaller symptom improvements 3
Maintenance Treatment:
Continue at the effective dose (typically 10-15 mg/day) and periodically reassess the need for ongoing treatment 1.
Bipolar I Disorder (Acute Manic/Mixed Episodes)
Start at 15 mg once daily for monotherapy, or 10-15 mg once daily when used adjunctively with lithium or valproate. 1 The target dose is 15 mg/day, which may be increased to 30 mg/day based on clinical response, though doses above 30 mg/day have not been evaluated for safety 1.
Major Depressive Disorder (Adjunctive Treatment)
Start at 2-5 mg/day when adding to an existing antidepressant. 1 The therapeutic range is 2-15 mg/day. Increase gradually in 5 mg increments at intervals of no less than one week 1.
Special Populations & Dose Adjustments
CYP2D6 Poor Metabolizers:
- Administer half the usual dose 1
- If also taking strong CYP3A4 inhibitors, reduce to one-quarter (25%) of the usual dose 1
Concomitant CYP Inhibitors:
- Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin): Reduce dose by 50% 1
- Strong CYP2D6 inhibitors (e.g., quinidine, fluoxetine, paroxetine): Reduce dose by 50% 1
- Combined strong CYP3A4 and CYP2D6 inhibitors: Reduce to 25% of usual dose 1
CYP3A4 Inducers:
- Strong CYP3A4 inducers (e.g., carbamazepine, rifampin): Double the usual dose, then reduce back to original level over 1-2 weeks when the inducer is discontinued 1
Older or Frail Patients:
When using aripiprazole for delirium management in cancer patients, reduce the dose in older patients and in poor CYP2D6 metabolizers 4. For delirium, the suggested starting dose is 5 mg PO or IM stat, given once every 24 hours if scheduled dosing is required 4.
Administration Details
- No food restrictions - can be taken with or without meals 1
- No titration required for most patients starting at standard doses 1
- Once-daily dosing is sufficient due to the long elimination half-life (approximately 75 hours for aripiprazole, 94 hours for the active metabolite) 2, 5
Common Pitfalls to Avoid
- Don't increase doses too quickly - wait at least 2 weeks between adjustments to allow steady-state to be reached 1
- Don't exceed 20 mg/day expecting better results - higher doses show diminishing returns and may worsen outcomes 3
- Don't forget to adjust for drug interactions - CYP2D6 and CYP3A4 interactions are clinically significant and require dose modifications 1
- Don't discontinue too early - allow 1-4 weeks for full therapeutic effect before judging efficacy 2, 6
Switching from Other Antipsychotics
When switching to aripiprazole from another antipsychotic, maintain the previous antipsychotic at a stable dose until response to aripiprazole is satisfactory, then taper the previous medication slowly over several weeks 7. Minimize the period of overlapping antipsychotic administration 1.
