Starting Dose of Aripiprazole (Abilify)
For adults with schizophrenia or bipolar mania, start aripiprazole at 10-15 mg once daily; for major depressive disorder augmentation, start at 2-5 mg daily. 1
Dosing by Indication
Schizophrenia (Adults)
- Starting dose: 10 or 15 mg once daily
- Target dose: 10-15 mg/day
- Effective range: 10-30 mg/day
- Timing: Once daily, without regard to meals
- Important: Wait at least 2 weeks before increasing dose (time to steady state) 1
The FDA label is clear that doses higher than 10-15 mg/day were not more effective in clinical trials 1. Research supports that 10 mg/day appears optimal, with the highest response rates seen at this dose 2. Doses above 20 mg/day provide no additional benefit and may actually show smaller symptom improvements 3, 2.
Bipolar I Disorder - Acute Mania (Adults)
- Monotherapy: Start at 15 mg once daily
- Adjunctive therapy (with lithium/valproate): Start at 10-15 mg once daily
- Target dose: 15 mg/day
- Maximum: May increase to 30 mg/day based on clinical response 1
UK consensus guidance supports starting at 15 mg/day (range 5-20 mg/day) for mania 4.
Major Depressive Disorder - Adjunctive Treatment (Adults)
- Starting dose: 2-5 mg/day
- Dosage range: 2-15 mg/day
- Titration: Increase by up to 5 mg/day at intervals of no less than one week 1
This lower starting dose is critical when augmenting antidepressants to minimize side effects while achieving therapeutic benefit.
Delirium Management (Cancer/Palliative Care)
- Starting dose: 5 mg oral or intramuscular stat
- Scheduled dosing: 5 mg once daily (every 24 hours) if needed
- Special populations: Reduce dose in older patients and poor CYP2D6 metabolizers 5
Aripiprazole is classified as a third-generation antipsychotic with lower risk of extrapyramidal symptoms compared to other agents 5.
Adolescents (13-17 years)
Schizophrenia
- Day 1-2: Start at 2 mg/day
- Day 3-4: Increase to 5 mg/day
- Day 5+: Target dose of 10 mg/day
- Subsequent increases: 5 mg increments as needed
- Note: 30 mg/day was not more effective than 10 mg/day 1
Bipolar I Disorder (Ages 10-17)
- Day 1-2: Start at 2 mg/day
- Day 3-4: Increase to 5 mg/day
- Day 5+: Target dose of 10 mg/day
- Subsequent increases: 5 mg increments if needed 1
Pediatric Dosing (Other Indications)
Irritability Associated with Autism (Ages 6-17)
- Starting dose: 2 mg/day
- Initial increase: 5 mg/day
- Target range: 5-15 mg/day
- Titration: Increase by up to 5 mg/day at intervals of no less than one week 1
Tourette's Disorder (Ages 6-18)
For patients <50 kg:
- Start at 2 mg/day with target of 5 mg/day after 2 days
- May increase to 10 mg/day if needed
For patients ≥50 kg:
- Days 1-2: 2 mg/day
- Days 3-7: 5 mg/day
- Day 8+: Target 10 mg/day
- May increase up to 20 mg/day in 5 mg increments weekly 1
Critical Dose Adjustments
CYP2D6 Poor Metabolizers
- Administer half the usual dose 1
Concomitant Strong CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin)
- Reduce dose to half 1
Concomitant Strong CYP2D6 Inhibitors (e.g., quinidine, fluoxetine, paroxetine)
- Reduce dose to half 1
Combined Strong CYP3A4 + CYP2D6 Inhibitors
- Reduce dose to one-quarter (25%) of usual dose 1
Strong CYP3A4 Inducers (e.g., carbamazepine, rifampin)
- Double the usual dose
- When inducer is withdrawn, reduce aripiprazole dose over 1-2 weeks 1
Key Clinical Considerations
Steady-state timing: Aripiprazole has a long half-life (approximately 75 hours), with steady-state reached in 14 days. Full therapeutic effect may take 1-4 weeks 6. This is why dose increases should not occur before 2 weeks 1.
Early side effects: Nausea, insomnia, and agitation occur in 10-20% of patients but typically resolve within 3-7 days 7. Consider adjunctive medications during the first week to ensure treatment completion 4, 7.
Switching strategies: When switching from another antipsychotic, maintain the therapeutic dose of current treatment while adding aripiprazole at the recommended starting dose. Only reduce the previous medication once an effective aripiprazole dose is reached, then taper slowly over several weeks 7.
Older/frail patients: Use lower starting doses and titrate gradually. In delirium management, this may mean starting at 2.5-5 mg rather than the standard dose 5.
