Prednisone Dose in DA-EPOCH Chemotherapy
The prednisone dose in dose-adjusted EPOCH (DA-EPOCH) is 60 mg/m² orally on days 1-5 of each cycle.
Standard Dosing Regimen
The DA-EPOCH regimen consists of:
- Prednisone: 60 mg/m² orally days 1-5 1
- Etoposide: continuous infusion over 96 hours
- Vincristine: continuous infusion over 96 hours
- Doxorubicin: continuous infusion over 96 hours
- Cyclophosphamide: IV bolus on day 5
- Rituximab: added for B-cell lymphomas (DA-EPOCH-R)
The cycle is repeated every 21 days, with dose adjustments of the chemotherapy agents based on nadir blood counts from the previous cycle 1, 2.
Key Clinical Points
Prednisone administration specifics:
- Given as a fixed dose based on body surface area (60 mg/m²)
- Administered orally for 5 consecutive days at the start of each cycle
- Unlike the other agents in EPOCH, prednisone is not dose-adjusted between cycles
- Should be taken in the morning to minimize HPA axis suppression 3
Important caveats:
- The prednisone component remains constant at 60 mg/m² regardless of dose adjustments made to the other chemotherapy agents
- This differs from CHOP regimens where prednisone is typically given as 100 mg flat dose or 40-60 mg/m² 4, 5
- Patients should not abruptly discontinue prednisone after completing therapy; gradual tapering may be considered if clinically indicated 3
The evidence supporting this dosing comes from multiple prospective studies, including the pivotal NCI phase II trial in MYC-rearranged lymphomas 1 and studies in peripheral T-cell lymphomas 2, all consistently using 60 mg/m² days 1-5.
