Cosentyx Dosing for Psoriatic Arthritis
For adults with psoriatic arthritis, start Cosentyx (secukinumab) at 150 mg subcutaneously at weeks 0,1,2,3, and 4, then every 4 weeks thereafter; if the patient has coexistent moderate-to-severe plaque psoriasis, use 300 mg instead, and for patients with inadequate response to anti-TNF therapy, start directly at 300 mg. 1, 2
Standard Dosing Algorithm
Subcutaneous Administration (Primary Route)
Initial dose selection based on clinical presentation:
PsA with moderate-to-severe plaque psoriasis: 300 mg at weeks 0,1,2,3,4, then every 4 weeks 1, 2
- Given as one 300 mg injection OR two 150 mg injections
PsA without significant skin involvement: 150 mg at weeks 0,1,2,3,4, then every 4 weeks 1, 2
- Can be given with or without the loading regimen (weeks 0-4)
- Without loading: 150 mg every 4 weeks from start
Anti-TNF inadequate responders: 300 mg at weeks 0,1,2,3,4, then every 4 weeks 1
- This higher dose is specifically recommended for this population
Dose escalation strategy:
- If patient continues to have active PsA on 150 mg, increase to 300 mg every 4 weeks 1, 2
- Evidence supports improved efficacy with dose escalation from 150 mg to 300 mg 3, 4
Intravenous Administration (Alternative Route)
For adults with PsA who prefer or require IV administration 2, 5, 6:
- With loading: 6 mg/kg IV at week 0, then 1.75 mg/kg IV every 4 weeks (maintenance)
- Without loading: 1.75 mg/kg IV every 4 weeks from start
- Administered as 30-minute infusion
- Maximum maintenance dose: Do not exceed 300 mg per infusion for the 1.75 mg/kg maintenance dose
The IV regimen was approved through model-informed drug development to match SC exposure profiles 6, 7. The 1.75 mg/kg maintenance dose provides steady-state exposure within the range of approved SC regimens.
Pediatric Dosing (Age ≥2 years with Juvenile PsA)
Weight-based subcutaneous dosing at weeks 0,1,2,3,4, then every 4 weeks 1, 2:
- ≥15 kg to <50 kg: 75 mg
- ≥50 kg: 150 mg
Key Clinical Considerations
Loading regimen benefits:
- The loading regimen (weeks 0,1,2,3,4) provides numerically higher and earlier responses for some high-hurdle endpoints compared to no loading 3
- Consider using loading regimen for patients requiring rapid disease control
When to use 300 mg over 150 mg:
- Coexistent moderate-to-severe plaque psoriasis (mandatory) 1, 2
- Anti-TNF inadequate responders (strongly recommended) 1
- Inadequate response to 150 mg (dose escalation) 2
- Meta-analysis shows 300 mg superior to 150 mg, especially in anti-TNF-IR patients 4
Response assessment:
- Consider discontinuation if no response after 16 weeks 1
- Primary efficacy typically assessed at week 16 5, 8
- Sustained improvements demonstrated through 52-104 weeks 3, 9
Important Safety Precautions
Before initiating treatment 2:
- Screen for active or latent tuberculosis (do not start with active TB)
- Complete age-appropriate vaccinations
- Avoid in patients with inflammatory bowel disease (can cause new cases or exacerbations) 1
Common pitfalls to avoid:
- Do not use in patients with active inflammatory bowel disease—secukinumab can worsen IBD 1
- Do not exceed 300 mg per infusion for IV maintenance dosing 2
- Avoid injection into areas with active psoriatic skin lesions 1
Combination therapy:
- May be administered with or without methotrexate 2
The evidence strongly supports secukinumab as effective therapy for PsA with ACR20 response rates of 50% at week 24 with both 150 mg and 300 mg regimens 8, and the safety profile remains consistent across dosing regimens with no significant increase in serious adverse events 4, 9.
