Dosing Cosentyx for Psoriatic Arthritis and Hidradenitis Suppurativa
For a patient with both psoriatic arthritis (PsA) and hidradenitis suppurativa (HS), start with secukinumab 300 mg subcutaneously at weeks 0,1,2,3, and 4, followed by 300 mg every 2 weeks as maintenance therapy.
Rationale for This Dosing Strategy
This recommendation addresses both conditions simultaneously by using the higher dose and more frequent maintenance interval:
For Psoriatic Arthritis Component
The 300 mg dose is superior to 150 mg for PsA, particularly when moderate-to-severe skin involvement coexists 1, 2. The prescribing information explicitly states: "For patients with concomitant moderate to severe plaque psoriasis see adult plaque psoriasis recommendation" which directs to 300 mg dosing 1.
- Standard PsA dosing without significant skin disease: 150 mg with loading (weeks 0-4) then every 4 weeks
- PsA with anti-TNF failure: 300 mg is recommended 1, 3
- PsA with coexistent moderate-to-severe psoriasis: 300 mg is the recommended dose 1, 3
The loading regimen (weekly for first month) provides faster onset compared to no loading regimen, with numerically higher early responses for high-hurdle endpoints 4.
For Hidradenitis Suppurativa Component
HS requires 300 mg dosing, and the every 2-week maintenance interval is critical for efficacy 1, 2. The FDA label and clinical trials are unequivocal on this point:
- Initial dose: 300 mg at weeks 0,1,2,3, and 4
- Maintenance: 300 mg every 4 weeks, BUT can be increased to 300 mg every 2 weeks based on clinical response 1, 2
The SUNSHINE and SUNRISE trials demonstrated that secukinumab 300 mg every 2 weeks was significantly more effective than every 4 weeks for HS 5. In SUNSHINE, the every-2-week regimen achieved 45% HiSCR response vs 34% placebo (p=0.0070), while every-4-weeks showed 42% (p=0.042, not meeting the stricter significance threshold). The every-2-week dosing showed sustained response through 52 weeks 5.
Administration Details
- Route: Subcutaneous injection
- Loading phase: 300 mg at weeks 0,1,2,3, and 4
- Maintenance: 300 mg every 2 weeks
- Formulation: Each 300 mg dose given as one 300 mg injection OR two 150 mg injections 1, 2
- Injection sites: Rotate between upper arms, thighs, or abdomen; avoid areas with active psoriasis 1
Monitoring and Adjustment Strategy
Assess response at 16 weeks 1, 2:
- If inadequate response in PsA: Continue 300 mg every 2 weeks (already at optimal dosing)
- If inadequate response in HS: Already at optimal frequency; consider discontinuation if no response
- Consider discontinuation if no response after 16 weeks 1
For the extension phase beyond 52 weeks, real-world data shows 75% of patients achieved HiSCR at week 48, with 72% achieving IHS4-55 6. However, the randomized withdrawal study showed that many patients maintained response even when meeting "loss of response" criteria 7.
Critical Safety Considerations
Pre-treatment screening mandatory 1, 2:
- Tuberculosis: Screen with TST and/or IGRA; treat latent TB before initiating secukinumab 1, 8
- Active infections: Contraindicated; do not start with active infection 1, 2
- Inflammatory bowel disease: Secukinumab is NOT recommended if IBD present; can cause new-onset or exacerbation 1
Common adverse effects 1:
- Upper respiratory infections (most common)
- Mucocutaneous candidiasis (mild-moderate, responsive to standard treatment)
- Headache (9-12% in trials)
- Neutropenia (usually mild and transient)
Important Clinical Pitfalls
- Do not use 150 mg maintenance for HS - this is inadequate dosing for this indication
- Do not use every-4-week maintenance initially for HS - start with every-2-weeks given superior efficacy demonstrated in trials
- Screen for IBD history - secukinumab can worsen or precipitate IBD, which would be particularly problematic in HS patients who may already have perianal involvement
- Avoid live vaccines during treatment 2
- Monitor for infections - particularly candidiasis and upper respiratory infections
Alternative Consideration
If the patient has anti-TNF inadequate response history, the 300 mg dose is even more strongly indicated for the PsA component 1, 9. Meta-analysis shows secukinumab 300 mg was significantly more effective than 150 mg in anti-TNF-IR patients 9.
