Neupogen Dosing
Neupogen (filgrastim) is dosed at 5 mcg/kg/day subcutaneously for most cancer patients receiving chemotherapy, started 1-3 days after chemotherapy and continued until the absolute neutrophil count (ANC) reaches 2,000-3,000 cells/μL. 1, 2
Standard Dosing by Clinical Setting
Cancer Patients on Myelosuppressive Chemotherapy
- Dose: 5 mcg/kg/day subcutaneously (preferred route) 1, 2
- Timing: Start 1-3 days after chemotherapy completion 1, 2
- Duration: Continue daily until ANC reaches 10,000/mm³ after the expected nadir, typically up to 2 weeks 2
- Dose escalation: May increase by 5 mcg/kg increments per cycle based on severity and duration of ANC nadir 2
- Administration options: SC bolus injection, short IV infusion (15-30 minutes), or continuous SC/IV infusion 2
Bone Marrow Transplant
- Dose: 10 mcg/kg/day 1, 2
- Timing: Start at least 24 hours after both chemotherapy AND bone marrow infusion 2
- Route: IV infusion (4 or 24 hours) or continuous 24-hour SC infusion 2
- Titration algorithm: 2
- When ANC >1,000/mm³ for 3 consecutive days → reduce to 5 mcg/kg/day
- If ANC remains >1,000/mm³ for 3 more days → discontinue
- If ANC drops <1,000/mm³ → resume at 5 mcg/kg/day
- If ANC drops <1,000/mm³ while on 5 mcg/kg/day → increase back to 10 mcg/kg/day
Peripheral Blood Progenitor Cell (PBPC) Mobilization
- Dose: 10 mcg/kg/day subcutaneously (higher than standard chemotherapy dosing) 1, 2
- Timing: Start 4 days before first leukapheresis 1, 2
- Duration: Continue through last leukapheresis procedure 1, 2
- Typical schedule: 6-7 days total, with leukapheresis on days 5,6, and 7 2
Severe Chronic Neutropenia
- Congenital neutropenia: 6 mcg/kg twice daily (BID) subcutaneously 2
- Idiopathic/cyclic neutropenia: Lower doses typically sufficient 2
Key Clinical Considerations
Monitoring Requirements
- Obtain CBC and platelet count before starting therapy 2
- Monitor CBC twice weekly during treatment 2
- For PBPC mobilization: Monitor neutrophil counts after 4 days and consider dose modification if WBC >100,000/mm³ 2
Common Pitfalls to Avoid
- Timing error: Never give filgrastim within 24 hours before chemotherapy—this can worsen myelosuppression 2
- Premature discontinuation: Continue through all chemotherapy cycles; stopping after only 2 cycles increases febrile neutropenia risk from 10% to 36% 1
- Inadequate duration: Don't stop too early—continue until ANC recovery to 10,000/mm³ post-nadir 2
- Single-use vials: Each vial/prefilled syringe is single-dose only; discard unused portions 2
Practical Administration Details
- Preferred route: Subcutaneous injection 1, 2
- Available formulations: 2
- 300 mcg/mL vials (1 mL)
- 600 mcg/mL prefilled syringes (0.5 mL = 300 mcg or 0.8 mL = 480 mcg)
- Storage: Refrigerate at 2-8°C; avoid shaking 2
- Needle safety: Prefilled syringes have UltraSafe® Needle Guards—activate after injection until audible click 2
Biosimilar Equivalence
Filgrastim-sndz and other biosimilars use identical dosing to reference filgrastim 1. Recent evidence confirms biosimilar Nivestim® achieves equivalent stem cell mobilization with the same 5 mcg/kg every 12 hours dosing 3.
Pediatric Considerations
The 5 mcg/kg/day dosing is appropriate for pediatric patients, with evidence supporting efficacy in reducing fever duration and hospital stays in high-risk neuroblastoma 4. For stem cell mobilization in pediatric donors, the standard adult dose of 5 mcg/kg every 12 hours for 4 days is effective 3.
