Contraindications for Filgrastim
Filgrastim is contraindicated in patients with a history of serious allergic reactions to human granulocyte colony-stimulating factors, including filgrastim products or pegfilgrastim products 1.
The Single Absolute Contraindication
This is the only formal contraindication listed in FDA labeling and represents an absolute prohibition on use. The contraindication stems from the risk of recurrent anaphylaxis, which can occur within days even after initial anti-allergic treatment has been discontinued 1.
What Constitutes a "Serious Allergic Reaction"?
Serious allergic reactions include:
- Anaphylaxis (the most critical concern)
- Severe hypersensitivity reactions requiring hospitalization
- Reactions necessitating permanent drug discontinuation
Important distinction: Minor reactions like non-serious urticaria (nettle rash) have been reported but do not constitute an absolute contraindication 2. The majority of serious allergic reactions occur upon initial exposure 1.
Critical Warnings That Are NOT Contraindications (But Require Extreme Caution)
While not absolute contraindications, the following conditions warrant serious consideration and may lead to drug discontinuation:
Sickle Cell Disorders
- Severe and sometimes fatal sickle cell crises can occur 1
- Discontinue filgrastim immediately if sickle cell crisis develops
- This is effectively a relative contraindication in practice
Active Conditions Requiring Immediate Discontinuation
If any of these develop during treatment, stop filgrastim immediately:
- Splenic rupture (including fatal cases reported) 1
- Acute respiratory distress syndrome (ARDS) 1
- Sickle cell crisis 1
- Capillary leak syndrome (hypotension, hypoalbuminemia, edema, hemoconcentration) 1
Common Pitfall to Avoid
Do not confuse common adverse effects with contraindications. Bone pain (occurs in ~82% of donors), headache, and fatigue are expected side effects that can be managed with oral analgesics 3. These do not constitute reasons to avoid or discontinue the drug unless they become severe or intolerable.
The evidence shows that filgrastim is generally well-tolerated, with most adverse events being transient and resolving within 7 days after discontinuation 3, 4. However, rare cases of prolonged musculoskeletal symptoms lasting months have been documented 5, though these represent complications rather than contraindications.
