Maximum Daily Dose of Plavix (Clopidogrel)
The standard maximum daily maintenance dose of clopidogrel is 75 mg once daily, though a higher maintenance dose of 150 mg daily for 6 days followed by 75 mg daily may be reasonable in select high-risk patients undergoing PCI who are not at high risk for bleeding. 1
Standard Dosing
The FDA-approved dosing for clopidogrel is straightforward 1:
- Acute Coronary Syndrome: 300 mg loading dose, then 75 mg once daily
- Recent MI/Stroke/PAD: 75 mg once daily (no loading dose)
Higher Maintenance Dosing in Specific Contexts
While 75 mg daily is the standard maximum, guidelines acknowledge that 150 mg daily maintenance dosing may be considered in carefully selected patients:
When 150 mg Daily May Be Reasonable:
The 2012 ACC/AHA guidelines provide a Class IIb recommendation for clopidogrel 600 mg loading dose followed by 150 mg daily for 6 days, then 75 mg daily in patients with definite UA/NSTEMI undergoing PCI as part of an early invasive strategy who are not at high risk for bleeding 2, 3.
Evidence Supporting Higher Dosing:
The CURRENT-OASIS 7 trial demonstrated that double-dose clopidogrel (150 mg daily for 7 days after a 600 mg loading dose) reduced cardiovascular events and stent thrombosis compared to standard dosing, though it increased major bleeding (1.6% vs 1.1%) 4. This represents the strongest evidence for higher maintenance dosing.
Important Caveats:
The 150 mg daily regimen is NOT standard practice and should only be considered in:
- High-risk ACS patients undergoing PCI
- Patients with low bleeding risk
- Limited duration (typically 6-7 days maximum before returning to 75 mg daily)
Do not exceed 150 mg daily maintenance dosing. Studies examining 225-300 mg daily doses were only for overcoming CYP2C19-related resistance and are not recommended for routine use 5.
Loading Doses vs. Maintenance Doses:
It's critical to distinguish loading from maintenance dosing:
- Loading doses up to 600 mg are well-established 6, 7
- Maintenance doses above 75 mg daily remain controversial and are not FDA-approved as standard therapy
In patients with documented clopidogrel resistance or CYP2C19 loss-of-function alleles, switching to prasugrel or ticagrelor is preferred over increasing clopidogrel dose 5, 8.
