What is Trulicity Used For?
Trulicity (dulaglutide) is a once-weekly injectable GLP-1 receptor agonist used to improve blood sugar control in adults and children 10 years and older with type 2 diabetes, and to reduce the risk of major cardiovascular events (heart attack, stroke, or cardiovascular death) in adults with type 2 diabetes who have established heart disease or multiple cardiovascular risk factors. 1
Primary Indications
Glycemic Control
Trulicity is FDA-approved as an adjunct to diet and exercise to improve glycemic control in:
- Adults with type 2 diabetes mellitus
- Pediatric patients 10 years of age and older with type 2 diabetes 1
The medication provides high to very high glucose-lowering efficacy with an average HbA1c reduction of approximately 1.4-1.6% 2. It works by stimulating insulin secretion and reducing glucagon secretion in a glucose-dependent manner 3.
Cardiovascular Risk Reduction
Trulicity is specifically indicated to reduce the risk of major adverse cardiovascular events (MACE) in adults with type 2 diabetes who have:
- Established cardiovascular disease, OR
- Multiple cardiovascular risk factors 1
The REWIND cardiovascular outcomes trial demonstrated that dulaglutide reduced MACE-3 (cardiovascular death, nonfatal MI, or nonfatal stroke) by approximately 12-14% compared to placebo, regardless of baseline HbA1c levels 4. This cardiovascular benefit is independent of glucose-lowering effects and should be considered even in patients at or near their glycemic targets 5, 2.
Additional Benefits Beyond Glucose Control
Weight Loss
Trulicity consistently produces moderate weight loss of 2-3 kg on average 6. This occurs through multiple mechanisms including slowing gastric emptying, promoting early satiety, reducing food intake, and decreasing appetite 4. The weight loss effect is sustained over time and represents an important advantage over insulin therapy 7.
Renal Protection
Trulicity has demonstrated renal benefits including:
- Prevention of new-onset macroalbuminuria
- Reduced urinary albumin excretion
- Slowed progression of eGFR decline 4
In patients with type 2 diabetes and chronic kidney disease, GLP-1 receptor agonists like dulaglutide reduce the risk of composite kidney outcomes by approximately 17-35%, primarily driven by effects on albuminuria 8, 9.
Clinical Positioning
First-Line Therapy Considerations
Current guidelines recommend prioritizing Trulicity (or other GLP-1 RAs with proven cardiovascular benefit) in specific patient populations independent of baseline HbA1c or metformin use 5, 2:
- Patients with established atherosclerotic cardiovascular disease
- Patients with multiple cardiovascular risk factors
- Patients with chronic kidney disease (eGFR 15-59 mL/min/1.73 m²) 8
- Patients with type 2 diabetes and obesity 4
Combination Therapy
Trulicity can be used:
- As monotherapy
- In combination with metformin
- With other oral antidiabetic agents
- As an alternative to basal insulin
- In combination with basal or prandial insulin 6, 7
When combined with insulin, Trulicity provides equivalent or superior HbA1c lowering with the advantages of weight loss (rather than weight gain) and minimal additional hypoglycemia risk 6, 7.
Important Limitations and Contraindications
Trulicity is NOT indicated for:
- Type 1 diabetes mellitus 1
- Patients with a personal or family history of medullary thyroid carcinoma (MTC) 1
- Patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2) 1
Use with caution or not recommended in:
- Severe gastrointestinal disease, including severe gastroparesis 1
- History of pancreatitis (consider alternative therapies) 6, 1
- End-stage renal disease (use with caution, though no dose adjustment required) 1
Dosing and Administration
Trulicity is administered as a once-weekly subcutaneous injection at any time of day, with or without food 1. The recommended starting dose is 0.75 mg weekly, with titration to 1.5 mg weekly or the maximally tolerated dose based on glycemic response and tolerability 5. The medication comes in a pre-filled, single-dose pen for ease of use 10.
Common Adverse Effects
The most frequent side effects are gastrointestinal: nausea (occurring in up to 25% of patients), vomiting, and diarrhea (up to 10% each) 4. These symptoms are typically transient, generally mild, and diminish over time with continued use 6. Slow dose titration can help mitigate these effects 5, 2.
Hypoglycemia risk is minimal when Trulicity is used alone or with metformin, but increases when combined with insulin or sulfonylureas 2, 1.
