Symbicort 160/4.5 µg Adult Dosing
For adults, Symbicort 160/4.5 µg is dosed as 2 inhalations twice daily (morning and evening, approximately 12 hours apart). 1
Asthma Dosing (Adults ≥12 years)
Standard maintenance dose: 2 inhalations of Symbicort 160/4.5 µg twice daily 1
- This represents the maximum recommended dosage for adults and adolescents ≥12 years with asthma 1
- Patients should rinse mouth with water after each use without swallowing 1
- Use a short-acting beta-agonist (SABA) for acute symptom relief between doses 1
Maintenance and Reliever Therapy (SMART) Option
For moderate-to-severe persistent asthma, an alternative evidence-based approach uses budesonide/formoterol 160/4.5 µg as both maintenance and reliever therapy 2, 3:
- Maintenance: 1-2 inhalations twice daily
- Reliever: Additional inhalations as needed for symptoms (up to maximum 12 total inhalations per day) 3
- This SMART regimen reduces severe exacerbations by 38-44% compared to fixed-dose ICS/LABA plus SABA 4, 5
- Achieves better asthma control at 30-40% lower total ICS exposure 6
Key advantage: The 2020 NAEPP guidelines give a strong recommendation for ICS-formoterol as maintenance and reliever therapy over higher-dose ICS alone or same-dose ICS-LABA plus SABA in patients ≥4 years with moderate-to-severe persistent asthma 2
COPD Dosing (Adults)
For COPD, only Symbicort 160/4.5 µg is indicated: 2 inhalations twice daily 1
Critical Warnings
- Never exceed 2 inhalations twice daily with standard fixed-dose therapy (higher doses increase adverse effects without additional benefit) 1
- Do not use additional LABA medications while on Symbicort 1
- Not for acute bronchospasm - use SABA instead 1
- Prime inhaler before first use (2 test sprays) and re-prime if unused >7 days 1
Dose Titration
If inadequate control after 1-2 weeks on Symbicort 80/4.5 µg, step up to 160/4.5 µg strength 1. If control fails on 160/4.5 µg, re-evaluate and consider adding oral corticosteroids or other adjunctive therapy rather than exceeding the maximum dose 1.
