Side Effects of Dupixent (Dupilumab) Autoinjector
The most common side effects of Dupixent are ocular surface problems (conjunctivitis and dry eyes occurring in 15-26% of patients), injection site reactions, upper respiratory tract infections, and cold sores, with the medication having an excellent overall safety profile requiring no routine laboratory monitoring. 1
Most Common Side Effects
According to the FDA label and clinical guidelines, the following side effects occur most frequently 1:
Very Common (>10% of patients):
- Injection site reactions - redness, swelling, pain at injection site
- Upper respiratory tract infections (common cold symptoms/nasopharyngitis)
- Eye and eyelid inflammation - including redness, swelling, itching, sometimes with blurred vision
- Conjunctivitis - occurs in 6-15% in clinical trials but up to 26% in real-world practice 2
- Dry eyes
Common (1-10% of patients):
- Cold sores in mouth or on lips (herpes virus infections)
- High count of certain white blood cells (eosinophilia)
- Back pain
- Inflammation inside the nose (rhinitis)
- Dizziness
- Muscle pain
- Diarrhea
- Throat pain
- Gastritis
- Joint pain (arthralgia)
- Trouble sleeping (insomnia)
- Toothache
- Headache
- Urinary tract infection
- Facial rash or redness
Serious Side Effects (Rare but Important)
Allergic Reactions
Stop Dupixent immediately and seek emergency care if you develop 1:
- Breathing problems or wheezing
- Fast pulse
- Swelling of face, lips, mouth, tongue, or throat
- Hives
- Fainting, dizziness, feeling lightheaded
- Severe joint pain
- Stomach cramps
Eye Problems Requiring Ophthalmology Referral
The 2024 expert consensus defines these as "dupilumab-related ocular surface disorders" (DROSD) 2. Use the RAPID acronym to identify when urgent ophthalmology referral (within 24 hours) is needed 2:
- Redness plus any of:
- Acuity loss or worsening vision
- Pain (moderate/severe ocular pain, not just irritation)
- Intolerance of light (photophobia)
- Damaged cornea visible or opacity
For most patients, conjunctivitis is self-limited and managed with preservative-free artificial tears 3. However, 4.2% of patients discontinue dupilumab due to ocular complications 2.
Joint Problems
Joint aches and pain can occur, with some patients experiencing trouble walking or requiring hospitalization 1. Your provider may stop Dupixent if severe joint symptoms develop.
Inflammation of Blood Vessels (Rare)
Occurs rarely in asthma patients, especially when oral steroids are being reduced. Watch for 1:
- Rash
- Worsening shortness of breath
- Persistent fever
- Chest pain
- Pins and needles or numbness in arms/legs
Real-World Side Effects Beyond Clinical Trials
Real-world use has revealed additional side effects not fully appreciated in trials 4, 5:
- Head and neck dermatitis (19.5% in one real-world study) 6
- Psoriasiform eruptions
- Inflammatory arthritis
- Seborrheic dermatitis
- Facial burning sensation
- Reported associations with alopecia areata, vitiligo, and weight gain
Key Safety Points
No Laboratory Monitoring Required
Dupixent requires no routine blood or urine testing before starting or during treatment 3, 7. Long-term studies up to 5 years show no clinically meaningful changes in laboratory parameters 7.
Excellent Long-Term Safety Profile
The medication has demonstrated an excellent safety track record over more than 5 years in clinical practice with few major emergent safety concerns 3. The overall safety profile remains consistent across all age groups from infants to elderly 8.
Management Algorithm for Side Effects
For injection site reactions: These are typically mild and self-resolve. Rotate injection sites with each dose 1.
For mild-to-moderate eye symptoms: Start preservative-free ocular lubricants immediately 2. Add topical antihistamine eyedrops if lubricants alone are ineffective 2.
For severe or persistent symptoms: Most side effects do not require discontinuation 6. However, dupilumab should be withdrawn if ophthalmology confirms progressive vision loss unresponsive to treatment or progressive conjunctival scarring 2.
Important Caveats
- Side effect rates in short-term studies were similar between dupilumab and placebo across all age groups 8
- There was no increase in side effects with long-term versus short-term treatment 8
- Most side effects are mild and do not require drug discontinuation 6
- The heterogeneity and variable onset time of adverse events warrant thorough patient history and both short- and long-term monitoring 5
