Laboratory Workup for Elevated Prolactin
When evaluating an elevated prolactin level, you must order TSH, creatinine, and a pregnancy test (in women of reproductive age) to exclude the three most common secondary causes: hypothyroidism, renal failure, and pregnancy 1.
Essential Laboratory Tests
The Endocrine Society guidelines provide a clear algorithmic approach for evaluating hyperprolactinemia 1:
First-Line Labs (Must Order)
- TSH - Primary hypothyroidism is a common reversible cause of hyperprolactinemia
- Creatinine/BUN - Renal insufficiency causes hyperprolactinemia in approximately one-third of patients with kidney disease due to decreased clearance and enhanced production 1
- Pregnancy test (β-hCG) - Mandatory in all women of reproductive age, as pregnancy is a physiological cause of elevated prolactin 1
Second-Line Testing (Based on Clinical Context)
For asymptomatic hyperprolactinemia:
- Macroprolactin screening via polyethylene glycol precipitation - Approximately 40% of patients with hyperprolactinemia have macroprolactinemia, which is less bioactive and may not require treatment 1. This test can eliminate unnecessary diagnostic workup and treatment.
When there's discordance between tumor size and prolactin level:
- Serial dilution of serum samples - If you see a very large pituitary tumor but only mildly elevated prolactin, order serial dilutions to rule out the "hook effect," a laboratory artifact in immunoradiometric assays that produces falsely low prolactin values 1
Critical Clinical Considerations
Medication Review
Before ordering any labs, thoroughly review all medications 1. Common culprits include:
- Antipsychotics (typical and atypical)
- Metoclopramide and domperidone
- Antidepressants (SSRIs, tricyclics)
- Antihypertensives (methyldopa, verapamil)
- H2 blockers
Common Pitfall to Avoid
Do not repeat prolactin measurements multiple times without clinical indication 2. A single elevated value should be confirmed with one repeat measurement drawn in the morning, fasting, without excessive venipuncture stress 1. The evidence shows substantial overutilization of repeated prolactin testing that doesn't lead to clinical action 2.
When Imaging is Indicated
After excluding secondary causes with the above labs, proceed directly to pituitary MRI (without gadolinium if pregnant) if hyperprolactinemia persists 1. Most patients with prolactin levels >250 µg/L will have a prolactinoma, and macroprolactinomas typically have levels >250 µg/L 1.
