Venofer Dosing Recommendations
For adult hemodialysis patients, administer Venofer 100 mg undiluted as a slow IV injection over 2-5 minutes or as an infusion over at least 15 minutes per consecutive hemodialysis session, with a usual total treatment course of 1000 mg. 1
Adult Dosing by CKD Population
Hemodialysis-Dependent CKD (HDD-CKD)
- Dose: 100 mg per hemodialysis session
- Administration:
- Undiluted slow IV injection over 2-5 minutes, OR
- Diluted in maximum 100 mL 0.9% NaCl over ≥15 minutes
- Timing: Early during dialysis session (generally within first hour)
- Total course: 1000 mg total
- Maintenance: 250-1000 mg within 12 weeks, frequency can be thrice weekly, twice weekly, weekly, or every other week 2, 1
Non-Dialysis Dependent CKD (NDD-CKD)
- Dose: 200 mg per administration
- Administration:
- Undiluted slow IV injection over 2-5 minutes, OR
- Diluted in maximum 100 mL 0.9% NaCl over 15 minutes
- Frequency: 5 different occasions over 14-day period (total 1000 mg)
- Alternative regimen (limited experience): 500 mg diluted in maximum 250 mL 0.9% NaCl over 3.5-4 hours on Day 1 and Day 14 1
Peritoneal Dialysis-Dependent CKD (PDD-CKD)
- Regimen: 3 divided doses within 28-day period:
- Two 300 mg infusions over 1.5 hours, 14 days apart
- One 400 mg infusion over 2.5 hours, 14 days after second dose
- Dilution: Maximum 250 mL 0.9% NaCl 1
Pediatric Dosing (≥2 Years of Age)
HDD-CKD (Maintenance Treatment)
- Dose: 0.5 mg/kg, not to exceed 100 mg per dose
- Frequency: Every 2 weeks for 12 weeks
- Administration:
- Undiluted slow IV injection over 5 minutes, OR
- Diluted in 0.9% NaCl at concentration 1-2 mg/mL over 5-60 minutes
- Important: Do not dilute to concentrations below 1 mg/mL 1
NDD-CKD or PDD-CKD on Erythropoietin (Maintenance Treatment)
- Dose: 0.5 mg/kg, not to exceed 100 mg per dose
- Frequency: Every 4 weeks for 12 weeks
- Administration: Same as HDD-CKD pediatric dosing 1
Note: Iron replacement treatment dosing in pediatric patients has not been established 1
Critical Safety Considerations
Monitoring Requirements
- Before administration: Monitor for hypersensitivity reactions
- During and after: Observe for at least 30 minutes until clinically stable 1
- Iron parameters: Monitor hemoglobin, hematocrit, serum ferritin, and transferrin saturation periodically
- Timing of labs: Do not perform serum iron measurements for at least 48 hours after IV dosing; wait 2-7 days after last dose for TSAT and ferritin depending on dose magnitude 2, 1
Contraindications and Warnings
- Absolute contraindication: Known hypersensitivity to Venofer 1
- Hypersensitivity reactions: Serious anaphylactic-type reactions can occur; only administer when personnel and therapies for treating serious reactions are immediately available 1
- Hypotension: Monitor for clinically significant hypotension; may be related to rate of administration and/or total dose 1
- Iron overload: Do not administer to patients with evidence of iron overload; excessive therapy can lead to iatrogenic hemosiderosis 1
Evidence-Based Context
The FDA-approved dosing 1 aligns with established guideline recommendations 2. Iron sucrose has demonstrated superior efficacy compared to oral iron in hemodialysis patients, with significant increases in hemoglobin levels evident after three doses and maintained for at least 5 weeks after completion 3. Studies show iron sucrose is well-tolerated with no serious adverse drug reactions when administered as 100 mg IV push without prior test dose 3.
Research comparing dosing strategies found that higher-dose proactive iron administration (median 1121 mg/month) was superior to lower-dose reactive administration (median 366 mg/month), resulting in lower erythropoiesis-stimulating agent requirements and fewer adverse events 4. However, when comparing iron sucrose 100 mg biweekly versus ferric carboxymaltose 500 mg every 10 weeks, the less frequent higher-dose regimen did not meet noninferiority for maintaining hemoglobin levels 5.
For peritoneal dialysis patients, even a low monthly maintenance dose of 100 mg iron sucrose has shown efficacy in improving hemoglobin levels and iron indices 6.
Treatment may be repeated if iron deficiency recurs across all patient populations 1.
