Dabigatran Hold Duration Before Fracture Repair

For fracture repair surgery, hold dabigatran for 2 days (skip 4 doses) if creatinine clearance (CrCl) ≥50 mL/min, or 4 days (skip 6-8 doses) if CrCl is 30-50 mL/min. 1, 2

Risk-Stratified Approach

Fracture repair, particularly hip fracture surgery, is considered a high bleeding risk procedure due to the need for complete hemostasis and potential for significant blood loss. The timing of dabigatran interruption depends critically on two factors:

1. Patient Renal Function (Most Critical Factor)

Dabigatran is 75-80% renally cleared, making renal function the primary determinant of drug elimination 3, 4:

  • CrCl ≥50 mL/min (normal/mild impairment):

    • Hold for 2 days before surgery (skip 4 doses)
    • This allows 4-5 half-lives (48-60 hours) to elapse 3, 1
  • CrCl 30-50 mL/min (moderate impairment):

    • Hold for 4 days before surgery (skip 6-8 doses)
    • Extended half-life of 16-18 hours requires 64-90 hours for adequate clearance 3, 1
  • CrCl <30 mL/min:

    • Hold for 4-5 days minimum
    • Consider measuring dabigatran levels if available 1, 2

2. Updated 2022 Guidelines vs. Older Recommendations

The 2022 CHEST guidelines 1 provide more streamlined recommendations compared to the 2012 Blood guidelines 3:

  • 2022 approach: 2 days off for high-bleed-risk if CrCl ≥50 mL/min; 4 days if CrCl <50 mL/min
  • 2012 approach: 3 days off (skip 4 doses) for normal renal function; 4-5 days for moderate impairment

The 2022 guidelines are more recent and based on the PAUSE trial 4, which demonstrated safe outcomes with shorter interruption periods in 668 dabigatran patients undergoing high-bleed-risk procedures (0.60% arterial thromboembolism rate, 0.9% major bleeding rate).

Critical Caveats

Do NOT Delay Surgery Unnecessarily

Recent evidence strongly suggests that delaying hip fracture surgery beyond medical optimization does not reduce transfusion rates 5, 6. Studies show:

  • No difference in transfusion rates between DOAC patients operated <24 hours vs. >24 hours 6
  • Early surgery (within 48 hours) is safe for DOAC patients 5
  • Median time to surgery was 20-42 hours in DOAC patients without increased complications 5, 6

Emergency/Urgent Surgery Considerations

If fracture repair cannot be delayed 2:

  • Measure dabigatran levels if available (target <30-50 ng/mL) 7, 8
  • Use idarucizumab (specific reversal agent) for emergency cases 2
  • Alternative: aPTT can help assess residual effect (though less sensitive) 9, 8
  • Prothrombin complex concentrates are NOT effective for dabigatran reversal 7

Neuraxial Anesthesia Warning

If spinal or epidural anesthesia is planned, longer interruption times are mandatory (up to 5 days for high-risk patients) due to catastrophic consequences of epidural hematoma 1, 10. The French guidelines 7, 10 strongly recommend against neuraxial procedures unless dabigatran concentration is documented <30 ng/mL.

Practical Algorithm

  1. Check CrCl immediately (use Cockcroft-Gault formula)
  2. Determine last dabigatran dose timing
  3. Apply hold duration:
    • CrCl ≥50: 2 days (48 hours)
    • CrCl 30-50: 4 days (96 hours)
    • CrCl <30: 4-5 days minimum
  4. If surgery cannot wait: Consider idarucizumab or measure levels
  5. Resume dabigatran: 48-72 hours post-operatively for high-bleed-risk surgery 1

Common Pitfall to Avoid

Do not automatically delay surgery 5 days for all dabigatran patients—this outdated approach increases hip fracture morbidity and mortality without reducing bleeding risk in patients with normal renal function 5, 6. The FDA label 2 supports 1-2 days for CrCl ≥50 mL/min and 3-5 days for CrCl <50 mL/min, aligning with contemporary evidence.
