Blistering Rash Causes
Blistering rashes arise from infections (viral/bacterial), autoimmune diseases (pemphigus, bullous pemphigoid), severe drug reactions (Stevens-Johnson syndrome/toxic epidermal necrolysis), metabolic disorders (porphyria), contact dermatitis, and genetic conditions (epidermolysis bullosa).
Primary Diagnostic Categories
The causes of blistering rashes can be systematically organized into several key categories based on mechanism and urgency:
Life-Threatening Causes (Require Immediate Recognition)
Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN) represents the most critical diagnosis to identify. These severe mucocutaneous reactions are usually triggered by drugs or infections and present with epidermal loss, multisite mucositis, and systemic symptoms 1. Key distinguishing features include:
- Prodrome of fever, malaise, and upper respiratory symptoms preceding the eruption by several days
- Cutaneous pain as a prominent early feature - this symptom should immediately alert you to incipient epidermal necrolysis
- Positive Nikolsky sign - minimal shearing forces cause epidermis to peel back
- Flaccid bullae with necrotic epidermis separating from dermis, producing sheets of detached epidermis
- Erosive and hemorrhagic mucositis affecting eyes, mouth, nose, and genitalia as an early feature 1
The mortality is lower in children than adults, but long-term sequelae can be severely debilitating 1.
Autoimmune Blistering Diseases
Pemphigus vulgaris causes extensive cutaneous erosions with fragility of normal skin (positive Nikolsky sign). The disease has significant mortality risk, particularly from infection and sepsis 2. These conditions require early dermatology referral as they can lead to significant morbidity and mortality if unrecognized and untreated 3.
Bullous pemphigoid is an inflammatory condition most commonly occurring in older adults, presenting as a progressive pruritic rash with fluid-filled blisters 4.
Other autoimmune causes include cicatricial pemphigoid, epidermolysis bullosa acquisita, dermatitis herpetiformis, and linear IgA bullous disease 5.
Infectious Causes
Bullous impetigo and other bacterial infections can cause blistering 3. In children, staphylococcal scalded skin syndrome (SSSS) is a common mimicker of TEN but is distinguished by:
- Absence of mucosal involvement (key differentiator from SJS/TEN)
- Intraepidermal cleavage (vs. subepidermal in SJS/TEN) 1
Viral infections like herpes simplex also commonly present with blisters 6.
Metabolic and Drug-Related Causes
Porphyria cutanea tarda represents a metabolic disease with cutaneous blistering manifestations 3, 5.
Drug eruptions including bullous fixed drug eruptions should always be considered in the differential 7.
Mechanical and Contact Causes
Burns, ischemia, and contact dermatitis frequently cause secondary blistering 6. These are among the most common causes encountered in practice.
Critical Diagnostic Approach
When evaluating blistering, assess:
- Age of onset - genetic conditions present in neonates; autoimmune disorders typically later in life 6
- Distribution, number, size, and morphology of blisters 8
- Mucosal involvement - presence suggests SJS/TEN or pemphigus; absence suggests SSSS or other causes 1
- Peribullous skin characteristics and evolution pattern 8
- Systemic symptoms - fever, pain, constitutional symptoms indicate severe disease 1
Skin biopsy with histopathology and immunofluorescence is usually required for accurate diagnosis of autoimmune blistering diseases 3. In cases of diagnostic uncertainty between SSSS and TEN, a frozen section of blister roof identifies the plane of cleavage 1.
Common Pitfalls
- Delaying diagnosis can result in severe or fatal consequences, particularly with autoimmune blistering diseases 5
- Confusing SSSS with TEN in children - always check for mucosal involvement 1
- Missing drug-induced causes - maintain high suspicion for medication triggers 7
- Underestimating infection risk - infection and sepsis are major causes of mortality in pemphigus vulgaris and require vigilant monitoring 2
