Vildagliptin Blood Glucose Reduction
Vildagliptin reduces fasting plasma glucose by approximately 1.0-1.7 mmol/L when added to metformin, and by 1.3 mmol/L as monotherapy.
Magnitude of Glucose Reduction
The glucose-lowering effect of vildagliptin varies based on how it's used:
As Add-on to Metformin
- Fasting plasma glucose (FPG) reduction: 0.8-1.7 mmol/L depending on dose 1
- 50 mg daily: -0.8 mmol/L
- 100 mg daily: -1.7 mmol/L
- Mean fasting plasma glucose reduction: -1.0 mmol/L when compared to placebo 2
- When added to insulin with or without metformin: -0.43 mmol/L 3
As Monotherapy
- FPG reduction: -1.3 mmol/L compared to baseline 4
- Mean plasma glucose reduction: 1.4-2.5 mmol/L depending on dose 5
- 25 mg twice daily: -1.4 mmol/L (10% reduction)
- 100 mg twice daily: -2.5 mmol/L (19% reduction)
Comparison to Other Agents
When compared head-to-head with glimepiride as add-on to metformin, vildagliptin produced comparable FPG reductions of -1.01 mmol/L versus -1.14 mmol/L with glimepiride 6.
Clinical Context
The glucose-lowering effect is dose-dependent and most pronounced with the 100 mg daily dose (50 mg twice daily). The reduction in fasting glucose correlates with improvements in HbA1c of approximately 0.7-1.1% when added to metformin 1.
Important Caveats
- The glucose reduction is modest compared to more potent agents like GLP-1 receptor agonists or SGLT2 inhibitors
- Current guidelines 7 do not specifically recommend DPP-4 inhibitors like vildagliptin as preferred agents for patients with cardiovascular disease, heart failure, or chronic kidney disease—where SGLT2 inhibitors and GLP-1 RAs are prioritized
- The primary advantage of vildagliptin is its very low hypoglycemia risk (1.7% incidence) 6 and weight neutrality
Practical Application
For a patient with baseline FPG of 10 mmol/L on metformin alone, adding vildagliptin 50 mg twice daily would be expected to reduce FPG to approximately 8.3-8.5 mmol/L based on the available evidence.
