FOLFIRI Dosing for Colorectal Cancer

The standard FOLFIRI regimen consists of irinotecan 180 mg/m² IV over 30-90 minutes on day 1, leucovorin 400 mg/m² IV over 2 hours on day 1,5-FU 400 mg/m² IV bolus on day 1, followed by 5-FU 1200 mg/m²/day × 2 days (total 2400 mg/m² over 46-48 hours) continuous infusion, repeated every 2 weeks 1, 2, 1, 3, 4.

Standard Dosing Components

The FOLFIRI regimen has been consistently defined across multiple NCCN and international guidelines:

  • Irinotecan: 180 mg/m² IV infusion over 30-90 minutes, day 1
  • Leucovorin (LV): 400 mg/m² IV infusion over 2 hours, day 1 (or levoleucovorin 200 mg/m²)
  • 5-FU bolus: 400 mg/m² IV bolus, day 1
  • 5-FU continuous infusion: 1200 mg/m²/day × 2 days (total 2400 mg/m² over 46-48 hours)
  • Cycle frequency: Every 2 weeks

Critical safety note: Chemotherapy orders should be limited to 24-hour units (1200 mg/m²/day, NOT 2400 mg/m² over 48 hours) to minimize medication errors 1, 2, 1.

Dose Adjustments for UGT1A1 Polymorphisms

For patients with known UGT1A1 genotypes, irinotecan dosing should be adjusted to optimize efficacy while minimizing toxicity:

UGT1A1 *1/*1 (Wild-type)

  • Recommended dose: 310 mg/m² irinotecan when combined with FOLFIRI + bevacizumab 5
  • These patients can tolerate higher doses than standard without increased toxicity 6, 5, 7
  • One study demonstrated 370 mg/m² was feasible in *1/*1 patients without bevacizumab 6

UGT1A1 *1/*28 (Heterozygous)

  • Recommended dose: 260-310 mg/m² irinotecan 6, 5
  • Standard dose of 180 mg/m² may be suboptimal for efficacy 6, 7
  • 310 mg/m² showed acceptable toxicity in some studies 6

UGT1A1 *28/*28 (Homozygous)

  • Recommended dose: 180 mg/m² or lower 6
  • These patients are at significantly increased risk for severe neutropenia 4
  • Consider dose reduction to 150 mg/m² or alternative regimens

Important caveat: The FDA label warns that patients with reduced UGT1A1 activity (particularly *28/*28 genotype) are at increased risk for severe neutropenia and should receive reduced initial doses 4. However, recent evidence suggests *1/*1 and *1/*28 patients may benefit from dose escalation 6, 5, 7.

Dose Modifications for Toxicity

Severe Diarrhea

  • Interrupt FOLFIRI and reduce subsequent doses if severe diarrhea occurs 4
  • Early diarrhea (during/shortly after infusion): Consider prophylactic atropine 0.25-1 mg IV/SC 4
  • Late diarrhea (>24 hours post-infusion): Treat promptly with loperamide, monitor fluid/electrolytes, institute antibiotic therapy if fever or severe neutropenia develops 4

Severe Myelosuppression

  • Interrupt treatment and reduce subsequent doses if severe neutropenia occurs 4
  • Most common dose-limiting toxicity across all studies 6, 5, 8
  • Grade 3-4 neutropenia occurred in 44-47.7% of patients at standard doses 8, 9

Hepatic Impairment

  • Irinotecan has not been adequately studied in patients with:
    • Serum bilirubin >2.0 mg/dL
    • Transaminases >3× ULN (without liver metastases)
    • Transaminases >5× ULN (with liver metastases) 1
  • Use with extreme caution in Gilbert's disease or elevated bilirubin 1

Special Populations

Elderly and Frail Patients

  • Sequential monotherapy starting with fluoropyrimidine alone remains a valid option in frail patients, as combination chemotherapy was not superior to sequential treatment for overall survival in the FOCUS and CAIRO trials 10, 11, 10
  • However, when objective response is the primary goal (e.g., for potential surgical resection), combination chemotherapy remains preferred 10, 11, 10

Japanese Patients

  • Recommended doses for Japanese patients without UGT1A1*28: Irinotecan 180 mg/m² and 5-FU 2400 mg/m² continuous infusion 8
  • These doses were well-tolerated with manageable toxicities 8

Combination with Biologics

FOLFIRI + Bevacizumab

  • Bevacizumab 5 mg/kg IV on day 1, every 2 weeks 2, 3
  • Bevacizumab does not alter irinotecan pharmacokinetics 5
  • This combination is a standard first-line option for metastatic colorectal cancer 2, 3

FOLFIRI + Cetuximab (KRAS wild-type only)

  • Cetuximab 400 mg/m² first infusion, then 250 mg/m² IV weekly 2
  • Alternative: Cetuximab 500 mg/m² IV every 2 weeks 2
  • Only for patients with KRAS wild-type tumors 2

FOLFIRI + Panitumumab (KRAS wild-type only)

  • Panitumumab 6 mg/kg IV over 60 minutes on day 1, every 2 weeks 2
  • Only for patients with KRAS wild-type tumors 2

Alternative Irinotecan-Based Regimens

CapIRI (Capecitabine + Irinotecan)

  • Irinotecan 180 mg/m² IV day 1, capecitabine 1000 mg/m² PO twice daily days 1-7, every 2 weeks 3
  • The original 3-weekly regimen (capecitabine 2000 mg/m²/day × 14 days + irinotecan 250 mg/m² q3weeks) is more toxic than FOLFIRI and less frequently used 10, 11, 10

Single-Agent Irinotecan

  • 125 mg/m² IV over 30-90 minutes on days 1,8,15,22, then 2-week rest 4
  • Alternative: 350 mg/m² IV over 90 minutes every 3 weeks 4
  • Recent data suggests single-agent irinotecan may have similar efficacy to FOLFIRI with better toxicity profile in second-line setting after FOLFOX failure 9

Key Clinical Pitfalls

  1. Medication error risk: Always order 5-FU continuous infusion as 1200 mg/m²/day for 2 days, NOT as 2400 mg/m² over 46-48 hours 1, 2, 1

  2. UGT1A1 testing: While genotype-guided dosing shows promise, routine testing is not universally recommended in guidelines 1. However, systematic reviews support dose escalation in *1/*1 and *1/*28 patients 7

  3. Bolus vs. infusional 5-FU: Infusional regimens are preferred and less toxic than bolus regimens 1. Bolus 5-FU regimens are inappropriate when combined with irinotecan 1

  4. Bevacizumab-specific toxicities: Monitor for hypertension, proteinuria, arterial thrombosis, mucosal bleeding, GI perforation, and wound healing complications 10

  5. Treatment duration: Maintenance with fluoropyrimidine alone after initial combination therapy prolongs progression-free survival compared to complete treatment break 10, 11, 10
