Doxepin 10 mg Indication

Doxepin 10 mg is FDA-approved for the treatment of depression and anxiety disorders, including psychoneurotic patients with depression and/or anxiety, depression/anxiety associated with alcoholism or organic disease, and psychotic depressive disorders with associated anxiety. 1

Primary FDA-Approved Indications

According to the FDA drug label, doxepin hydrochloride capsules are specifically indicated for:

  • Psychoneurotic patients with depression and/or anxiety
  • Depression and/or anxiety associated with alcoholism (not to be taken concomitantly with alcohol)
  • Depression and/or anxiety associated with organic disease
  • Psychotic depressive disorders with associated anxiety, including involutional depression and manic-depressive disorders 1

The target symptoms that respond particularly well include anxiety, tension, depression, somatic symptoms and concerns, sleep disturbances, guilt, lack of energy, fear, apprehension, and worry 1.

Dosing Context for 10 mg

The 10 mg dose represents a low starting dose in the FDA-approved dosing range. For most patients with mild to moderate illness severity, the recommended starting daily dose is 75 mg, with an optimum range of 75-150 mg/day 1. However, in patients with very mild symptomatology or emotional symptoms accompanying organic disease, lower doses may suffice, with some patients controlled on doses as low as 25-50 mg/day 1.

Important Clinical Distinction: Off-Label Use for Insomnia

While the 10 mg dose is not FDA-approved for insomnia treatment, it's crucial to note that low-dose doxepin (3 mg and 6 mg) has separate FDA approval specifically for insomnia 2. The American Academy of Sleep Medicine (2017) recommends doxepin for treating sleep maintenance insomnia, showing mean improvements of 26-32 minutes in total sleep time and 22-23 minutes reduction in wake after sleep onset compared to placebo 2. However, these recommendations are based on 3 mg and 6 mg doses, not the 10 mg formulation 2.

Critical Safety Warnings

Black Box Warning: Doxepin carries an FDA black box warning regarding increased risk of suicidal thinking and behavior in children, adolescents, and young adults with major depressive disorder and other psychiatric disorders 1. All patients started on doxepin require close monitoring for clinical worsening, suicidality, or unusual behavioral changes.

Key Contraindications:

  • Hypersensitivity to doxepin or other dibenzoxepines
  • Glaucoma or tendency to urinary retention 1
  • Not approved for pediatric use under 12 years of age 1
