Metrogel for Inflammatory Rosacea
Metrogel (metronidazole 0.75% or 1% gel) is FDA-approved and effective for treating inflammatory papules and pustules of rosacea, applied once daily to affected areas after cleansing. 1
Indication
Metrogel is specifically indicated for the topical treatment of inflammatory lesions (papules and pustules) of rosacea 1. It functions as both an antimicrobial and anti-inflammatory agent by inhibiting inflammatory mediators generated by neutrophils 2.
Dosing Regimen
Apply once daily:
- Cleanse the affected areas before application
- Apply and rub in a thin film to affected area(s) once daily
- Cosmetics may be applied after Metrogel application
- For topical use only—not for oral, ophthalmic, or intravaginal use 1
Both 0.75% and 1% concentrations are equally effective when used once daily 2, 3. The evidence shows no significant difference in efficacy between these two strengths, with both producing approximately 60-65% reduction in inflammatory lesion counts over 12 weeks 2, 3.
Efficacy Profile
Clinical improvement timeline:
- Initial improvement visible as early as 3 weeks 2
- Optimal results achieved by 9-12 weeks of continuous therapy 2, 4
Expected outcomes at 12 weeks:
- 65% decrease in inflammatory lesion counts (versus 15% with vehicle) 2
- 57-62.5% of patients achieve "clear to mild" or "improved/much improved" status 2, 3
- In Japanese patients, 72.3% achieved >50% reduction in inflammatory lesions with improved erythema (versus 36.9% with vehicle) 4
Long-term efficacy: A 6-month double-blind study confirmed sustained efficacy and safety for long-term control of inflammatory lesions 2. Maintenance therapy with metronidazole decreases relapses and extends intervals between flares 5.
Safety Profile
Common adverse reactions (>2% incidence):
- Upper respiratory tract infection (2.5%)
- Headache (2.2%)
- Influenza (1.4%)
- Contact dermatitis (1.3%)
- Hypertension (1.1%) 1
Local cutaneous reactions (worse than baseline):
- Dryness: 25.4% (mostly mild)
- Scaling: 24.6% (mostly mild)
- Pruritus: 15.8%
- Stinging/burning: 10.3% 1
Important safety warnings:
Neurologic concerns: Peripheral neuropathy (numbness/paresthesia) has been reported with topical metronidazole post-approval. Immediately reevaluate therapy if abnormal neurologic signs appear 1
Blood dyscrasias: Use with caution in patients with evidence or history of blood dyscrasia, as metronidazole is a nitroimidazole 1
Eye irritation: Avoid contact with eyes—topical metronidazole can cause tearing 1
Contact dermatitis: Both irritant and allergic contact dermatitis have been reported; discontinue if dermatitis occurs 1
Drug interactions: Oral metronidazole potentiates anticoagulant effects of coumarin/warfarin. Use caution when administering Metrogel to patients on anticoagulant therapy 1
Contraindications
Absolute contraindication: History of hypersensitivity to metronidazole or any ingredient in the formulation 1
Clinical Context
Position in treatment algorithm: According to the 2017 ROSCO guidelines, metronidazole is a first-line topical option for mild-to-moderate inflammatory papules/pustules of rosacea 6. While newer agents like ivermectin 1% cream and encapsulated benzoyl peroxide 5% show somewhat higher efficacy rates (38-50% IGA success versus 37-62.5% for metronidazole), metronidazole remains a well-established mainstay therapy with decades of safety data 2.
Combination therapy: For moderate-to-severe disease, metronidazole can be combined with oral doxycycline 40 mg (subantimicrobial dose) for enhanced efficacy 5. Multiple cutaneous features can be treated simultaneously with more than one agent 6.
Common pitfall: Patients may discontinue therapy prematurely before the 3-week mark when initial improvement becomes visible. Counsel patients that optimal results require 9-12 weeks of consistent use 2.
Lactation: Breastfeeding is not recommended while using Metrogel 1.
