Acute Management of Acute Inflammatory Demyelinating Polyneuropathy (AIDP)
Initiate intravenous immunoglobulin (IVIg) at 0.4 g/kg body weight daily for 5 consecutive days as first-line treatment for AIDP, the most common form of Guillain-Barré syndrome. 1
Treatment Selection
IVIg and plasma exchange are equally effective for treating AIDP, but IVIg is the preferred treatment because it is easier to administer and more widely available 1. Plasma exchange (200-250 mL plasma/kg body weight over five sessions) remains an equally valid alternative if IVIg is unavailable or contraindicated 1.
Critical Points About Treatment:
- Start treatment within 2 weeks of weakness onset for IVIg, or within 4 weeks for plasma exchange 2
- Do NOT use corticosteroids alone - eight randomized controlled trials showed no benefit, and oral corticosteroids actually worsened outcomes 1
- Do NOT combine plasma exchange followed by IVIg - this is no more effective than either treatment alone 1
- No other drugs or procedures have proven effective 1
Immediate Assessment for ICU Admission
Admit to ICU if any of the following are present 1:
Respiratory distress indicators:
- Breathlessness at rest or during talking
- Inability to count to 15 in one breath
- Use of accessory respiratory muscles
- Vital capacity <15-20 mL/kg or <1 liter
- Abnormal arterial blood gas or pulse oximetry
Severe autonomic dysfunction (arrhythmias, marked blood pressure variation)
Severe bulbar dysfunction (swallowing problems, diminished cough reflex)
Rapid progression of weakness
Respiratory Monitoring
Monitor all patients closely for respiratory failure, as it can occur without dyspnea symptoms 1. Up to 22% of GBS patients require mechanical ventilation within the first week 1. Use the Erasmus GBS Respiratory Insufficiency Score (EGRIS) to calculate the probability (1-90%) that a patient will need ventilation within 1 week 1.
Risk factors for prolonged mechanical ventilation include inability to lift arms from bed at 1 week post-intubation and axonal subtype on electrophysiology - consider early tracheostomy in these patients 1.
Common Pitfalls to Avoid
The mortality rate remains 3-10% even with optimal care 1, primarily due to autonomic involvement and respiratory complications. Do not wait for electrophysiological confirmation or CSF results to initiate treatment - both can be normal early in the disease course 1. The disease progresses rapidly, with most patients reaching maximum disability within 2 weeks 1.
Treatment-related fluctuations (TRF) occur when patients deteriorate after initial stabilization on therapy - this may require repeat IVIg treatment 2, though evidence for efficacy of repeat treatment remains uncertain 1.
