Maximum Dose of Unasyn (Ampicillin-Sulbactam)
For adults with normal renal function, the maximum dose of Unasyn is 12 grams per day total (8 grams ampicillin + 4 grams sulbactam), administered as 3 grams (2 grams ampicillin + 1 gram sulbactam) every 6 hours, with the critical limitation that sulbactam should not exceed 4 grams per day. 1
Standard Dosing Framework
The FDA-approved dosing range for adults is:
- Minimum: 1.5 g (1 g ampicillin + 0.5 g sulbactam) every 6 hours
- Maximum: 3 g (2 g ampicillin + 1 g sulbactam) every 6 hours
- Absolute sulbactam ceiling: 4 grams per day 1
Administration Details
Route and timing considerations:
- IV administration over 10-15 minutes as slow injection, or 15-30 minutes as infusion in 50-100 mL compatible diluent
- Can be given IM by deep injection
- The every-6-hour interval is standard; do not increase frequency beyond this 1
Critical Dosing Limitations
The sulbactam component is the rate-limiting factor. Even though ampicillin could theoretically be dosed higher, you cannot exceed 4 grams of sulbactam daily. This means:
- Maximum of four 1-gram sulbactam doses per day
- Corresponds to maximum of four 3-gram Unasyn doses (each containing 2 g ampicillin + 1 g sulbactam)
- Total daily maximum: 8 g ampicillin + 4 g sulbactam = 12 g Unasyn 1
Special Population: Pediatrics
For children ≥1 year old:
- 300 mg/kg/day total (200 mg/kg ampicillin + 100 mg/kg sulbactam) divided every 6 hours
- Children ≥40 kg: use adult dosing with same 4-gram sulbactam daily limit 1
Common Pitfall to Avoid
Do not attempt to exceed the 4-gram sulbactam limit even in severe infections. Recent PK/PD data shows that while high-dose extended-infusion regimens (3 g q8h over 4 hours) provide better target attainment for intermediate-susceptibility organisms, this still respects the 4-gram sulbactam daily maximum (9 grams total daily dose) 2. The FDA label explicitly caps sulbactam at 4 grams daily regardless of infection severity 1.
Renal Adjustment Context
While not asked, note that this maximum applies only to normal renal function (CrCl ≥30 mL/min). Below this threshold, both frequency and dose require reduction since ampicillin and sulbactam are renally cleared with similar kinetics 1.
