What to Check When Liver Enzymes Are Elevated
When liver enzymes are elevated, immediately determine the pattern of injury (hepatocellular vs cholestatic) and then systematically evaluate for the most common causes: non-alcoholic fatty liver disease (NAFLD), alcohol-related liver disease (ARLD), viral hepatitis, and medication/supplement toxicity.
Initial Pattern Recognition
First, classify the pattern of liver injury based on which enzymes are elevated 1:
- Hepatocellular pattern: Disproportionate elevation of ALT and AST compared to alkaline phosphatase
- Cholestatic pattern: Disproportionate elevation of alkaline phosphatase compared to ALT and AST
- Mixed pattern: Both enzyme groups elevated
The degree of elevation matters—mild elevations (<5× upper limit of normal) are most common in primary care and affect approximately 10% of the U.S. population 2.
First-Line Evaluation
History and Risk Factor Assessment
Obtain specific information about 3, 1, 2:
- Metabolic risk factors: Measure waist circumference, blood pressure, fasting lipids, fasting glucose or HbA1c (for NAFLD assessment)
- Alcohol intake: Use AUDIT score—scores >19 indicate alcohol dependency requiring referral to alcohol services 3
- Medications and supplements: All prescribed, over-the-counter medications, and herbal/dietary supplements
- Viral hepatitis risk factors: Injection drug use, transfusions, sexual exposure
Initial Laboratory Testing
For hepatocellular pattern, order 1, 2:
- Complete blood count with platelets
- Hepatitis A, B, and C serologies (anti-HAV IgM, HBsAg, anti-HCV)
- Iron studies (serum iron, total iron-binding capacity, ferritin) for hereditary hemochromatosis
- Serum albumin and prothrombin time/INR (assess hepatic function)
- Autoimmune markers if clinically indicated (ANA, anti-smooth muscle antibody, immunoglobulins)
- Ceruloplasmin and 24-hour urine copper (Wilson's disease, especially if age <40)
- Alpha-1 antitrypsin level and phenotype
For cholestatic pattern, order 1:
- Antimitochondrial antibody (primary biliary cholangitis)
- Abdominal imaging (ultrasound or MRCP for biliary obstruction/primary sclerosing cholangitis)
- Fractionated bilirubin (elevated conjugated bilirubin confirms hepatic origin)
Imaging
Obtain abdominal ultrasound to assess 4:
- Liver morphology and steatosis
- Biliary dilatation
- Portal hypertension features
Risk Stratification for Fibrosis
This is critical and often overlooked. Normal liver enzymes do not exclude advanced fibrosis or cirrhosis 3.
For NAFLD patients specifically:
Use FIB-4 or NAFLD Fibrosis Score as first-line risk stratification 3. These should be calculated automatically in primary care systems.
- FIB-4 calculation: Age × AST / (platelet count × √ALT)
- If elevated, proceed to second-line testing with serum ELF measurements or transient elastography (Fibroscan/ARFI)
For harmful drinkers:
Risk stratify using clinical assessment and Fibroscan/ARFI elastography 3. The NICE guideline recommends elastography for men drinking >50 units/week and women >35 units/week to detect cirrhosis and advanced fibrosis.
Common Pitfalls and Caveats
Do not assume NAFLD is the only diagnosis in obese patients—25% of the population drinks more than recommended guidelines, and there is synergy between alcohol and obesity (BMI >35 doubles liver disease risk at any alcohol intake) 3.
Autoimmune liver disease may present without autoantibodies or elevated immunoglobulins—entirely treatable conditions can be missed 3.
Extrahepatic causes exist: thyroid disorders, celiac disease, hemolysis, and muscle disorders can cause mildly elevated transaminases 2.
When to Refer
Refer to gastroenterology/hepatology when 3:
- Abnormal liver tests persist despite negative extended workup and no NAFLD risk factors
- Evidence of advanced liver disease (clinical features of cirrhosis or portal hypertension)
- Elevated ALT/AST with prolonged INR and elevated bilirubin (urgent evaluation needed) 4
- AUDIT score >19 (refer to alcohol services concurrently)
- Patients with cirrhosis require screening for esophageal varices and hepatocellular carcinoma
If Initial Workup is Unremarkable
For patients with metabolic risk factors consistent with NAFLD and otherwise negative workup, initiate lifestyle modification trial 2. If elevation persists, proceed with hepatic ultrasonography and testing for uncommon causes (Wilson's disease, alpha-1 antitrypsin deficiency, autoimmune hepatitis).
Consider liver biopsy only when 1:
- Serologic testing and imaging fail to establish diagnosis
- Staging is required
- Multiple diagnoses are possible
The relationship between alcohol and cirrhosis is exponential: at 20 units/week the relative risk is approximately 3, whereas at 80 units/week it is 30 3. Recent evidence shows AST and GGT are stronger predictors of major adverse liver outcomes than ALT, with AST >2× upper reference limit conferring >5% 10-year risk 5.
