Treatment of Giardiasis
For a patient with diarrhea and a positive culture for Giardia, tinidazole is the first-line treatment of choice. 1
First-line Treatment Options
Tinidazole
Alternative First-line: Nitazoxanide
- Dosage: 500mg orally twice daily with food for 3 days (adults and children ≥12 years) 4
- Advantages:
Second-line Treatment Option
Metronidazole
- Dosage: 250-750mg three times daily for 5-7 days 1
- Limitations:
Treatment Algorithm
Confirm diagnosis: Positive stool culture for Giardia lamblia
Assess patient factors:
- Age (tinidazole only approved for ≥3 years)
- Pregnancy status (avoid if possible)
- Immunocompetence (may require longer treatment course if immunocompromised)
- Ability to tolerate oral medications
Select appropriate therapy:
- First choice: Tinidazole single dose
- If tinidazole unavailable: Nitazoxanide for 3 days
- If both unavailable: Metronidazole for 5-7 days
Special Considerations
Immunocompromised Patients
- May require longer treatment courses 1
- Consider combination therapy in refractory cases
- Monitor closely for treatment failure
Treatment Failures
- Consider retreatment with a different agent
- Albendazole (400mg daily for 5 days) has shown comparable efficacy to metronidazole with fewer side effects 5

Clinical Pearls and Pitfalls
- Important: Treat symptomatic patients; treatment of asymptomatic carriers is generally not recommended 6
- Ensure adequate hydration during treatment, especially with persistent diarrhea
- Metronidazole has a high frequency of gastrointestinal side effects which may compound existing symptoms 1
- Single-dose tinidazole offers better compliance than multi-day regimens
- Consider testing and treating household contacts with symptoms
- Purification of water supply is an important preventive measure 6
Monitoring
- Follow-up stool examination may be considered 2-4 weeks after treatment if symptoms persist
- Most patients respond rapidly to appropriate therapy
- Consider alternative diagnoses if symptoms persist despite appropriate treatment
