Ear Drops for Perforated Tympanic Membrane
For patients with perforated tympanic membranes, non-ototoxic topical preparations such as ofloxacin otic drops should be prescribed to prevent iatrogenic hearing loss. 1
Recommended Ear Drops
First-Line Treatment
- Ofloxacin otic drops:
- Dosage for patients 12 years and older: Ten drops (0.5 mL) instilled into the affected ear twice daily for fourteen days 2
- Dosage for children 1-12 years old: Five drops (0.25 mL) instilled into the affected ear twice daily for ten days 2
- Ofloxacin has been shown to be safe and does not delay tympanic membrane healing compared to saline 3
Administration Technique
- Warm the solution by holding the bottle in hand for 1-2 minutes to prevent dizziness
- Position patient with affected ear upward
- Instill the drops into the ear canal
- Pump the tragus 4 times to facilitate penetration into the middle ear
- Maintain position for 5 minutes 2
Rationale for Non-Ototoxic Preparations
The American Academy of Otolaryngology-Head and Neck Surgery strongly recommends non-ototoxic topical preparations for perforated tympanic membranes to avoid potential sensorineural hearing loss 1. This recommendation is based on:
- Ototoxic preparations (containing aminoglycosides like neomycin) can cause permanent sensorineural hearing loss when they reach the middle ear through a perforation
- Research shows repeated doses (≥2 prescriptions) of neomycin eardrops are associated with increased risk of sensorineural hearing loss in patients with non-intact tympanic membranes 4
- Non-ototoxic fluoroquinolone drops (like ofloxacin) are more effective than systemic antibiotics for infection management 5
Evidence on Healing Effects
Different topical preparations have varying effects on tympanic membrane healing:
- Ofloxacin does not delay healing compared to saline control 3
- Ciprofloxacin delays healing more than ofloxacin 3
- Steroid-containing drops (dexamethasone) further delay healing 3
- Combination drops (ciprofloxacin + dexamethasone) show the most significant delay in healing 3
Special Considerations
For Fungal Infections
- For otomycosis with perforated tympanic membrane, clotrimazole solution applied with Q-tips has shown equal efficacy to physician-inserted medicated wicks, with better patient satisfaction and lower recurrence rates 6
For Traumatic Perforations
- Moist therapy with ofloxacin drops can improve closure rates and shorten healing time for large traumatic perforations 7
- Most traumatic perforations (especially in children) heal spontaneously within 6-8 weeks 8
Important Cautions
- Avoid ear drops containing aminoglycosides (neomycin, gentamicin) due to ototoxicity risk
- During active drainage (otorrhea), water precautions should be implemented until discharge resolves 1
- If no improvement occurs within 48-72 hours of treatment, reassessment is necessary to confirm diagnosis and exclude other causes 1
