Grading of Anterolisthesis
The grading of anterolisthesis, also known as anterior vertebral displacement, is typically rated from 1-4 depending on the severity of the slip 1. This grading system is used to classify the degree of vertebral body anterior displacement.
Meyerding Grading System
The Meyerding grading system is commonly used to grade spondylolisthesis, which includes anterolisthesis 2. This system grades the slip as:
- Grade I: 0-25% slip
- Grade II: 26-50% slip
- Grade III: 51-75% slip
- Grade IV: 76-100% slip
- Grade V: more than 100% slip, also known as spondyloptosis
Classification of Spondylolisthesis
Five major categories have been proposed for the classification of spondylolisthesis, including isthmic, degenerative, congenital, traumatic, and pathological types 1.
Key Points
- Anterolisthesis is graded from 1-4 depending on the severity of the slip 1
- The Meyerding grading system is used to classify the degree of vertebral body anterior displacement 2
- Spontaneous fusion of L5/S1 spondylolisthesis can occur with conservative management 3
- High-grade spondylolisthesis is defined as cases with more than 50% displacement and spondylolisthesis with Meyerding grade III and higher 2
- Various surgical techniques have been described for high-grade spondylolisthesis, including in-situ fusion, partial reduction and fusion, and complete reduction 2
