From the Guidelines

Multaq (dronedarone) does not contain iodine, as it is a non-iodinated derivative of amiodarone. This is a crucial distinction because the presence of iodine in amiodarone is associated with various side effects, particularly thyroid dysfunction 1. The development of dronedarone as an iodine-free alternative aims to mitigate these risks while maintaining efficacy in managing atrial fibrillation.

Key Characteristics of Dronedarone

  • Dronedarone is a class III antiarrhythmic indicated for reducing the risk of hospitalization for atrial fibrillation in patients with a history of paroxysmal or persistent atrial fibrillation 1.
  • It is available only for oral administration, with its absolute bioavailability increased 2- to 3-fold when taken with food, especially a high-fat meal 1.
  • Peak plasma concentrations are reached within approximately 3 to 6 hours, and steady state is achieved within 4 to 8 days, with an elimination half-life of 13 to 19 hours 1.
  • Dronedarone is extensively metabolized by CYP3A4 into more than 30 metabolites, most of which are inactive, with the N-debutyl metabolite being weakly active 1.

Clinical Implications

  • The absence of iodine in dronedarone makes it a potentially safer option for patients with thyroid disorders or those who have experienced iodine-related side effects with amiodarone.
  • However, dronedarone has its own set of potential side effects and contraindications, including liver problems and an increased risk of death in certain patients with severe heart failure.
  • The choice between dronedarone and other antiarrhythmic medications, such as amiodarone, should be based on a thorough assessment of the patient's medical history, current health status, and the specific risks and benefits associated with each medication 1.

From the FDA Drug Label

Dronedarone HCl is a benzofuran derivative with the following chemical name: N-{2-butyl-3-[4-(3-dibutylaminopropoxy)benzoyl]benzofuran-5-yl} methanesulfonamide, hydrochloride. Its empirical formula is C31H44N2O5 S, HCl with a relative molecular mass of 593. 2. The inactive ingredients are: Core of the tablets: Colloidal silicon dioxide, crospovidone, hypromellose, lactose monohydrate, magnesium stearate, poloxamer 407, starch Coating/polishing of the tablets: Carnauba wax, hypromellose, polyethylene glycol 6000, titanium dioxide.

The chemical formula of dronedarone does not contain iodine. The inactive ingredients also do not list iodine or any iodine-containing compounds. Therefore, Multaq (dronedarone) does not contain iodine 2.

From the Research

Composition of Multaq (Dronedarone)

  • Multaq (dronedarone) does not contain iodine, as stated in studies 3, 4, 5, 6
  • The absence of iodine in dronedarone is a key difference from amiodarone, which has high iodine content and is associated with thyroid dysfunction 5, 6
  • Dronedarone's structure is modified to lack the iodine moiety, reducing the risk of iodine-related adverse effects 4, 5, 6

Comparison with Amiodarone

  • Amiodarone contains high amounts of iodine, which can lead to thyroid dysfunction, including hypothyroidism and thyrotoxicosis 5, 6
  • Dronedarone, on the other hand, has a lower risk of thyroid-related adverse effects due to its lack of iodine content 3, 4, 5, 6
  • While dronedarone may be a safer alternative to amiodarone, it may also be less effective in some cases 4, 7
