Duration Controversy and Evidence Quality
While IDSA/ESCMID recommend 5 days 1, 2, UK guidelines promote 3-day courses 6. The evidence for 3-day regimens is weak and not well-supported by direct comparative trials 6. A placebo-controlled trial showed significant benefit at both 3 and 7 days with nitrofurantoin 100 mg four times daily for 3 days, but this dosing differs from current recommendations 7.
