Size Parameters
- Popliteal lymph nodes are considered abnormal when palpable, as these nodes are located deep in the subcutaneous tissue and should not be easily felt during routine examination 1
- On MRI imaging, normal popliteal lymph nodes measure approximately 3.6-4.1 mm in their smallest dimension in patients with osteoarthritis (representing a more normal baseline state) 2
- The general threshold for abnormal peripheral lymph nodes is greater than 1 cm in diameter, though popliteal nodes specifically are considered abnormal if palpable at any size 3, 4
Texture and Morphologic Features
- Normal popliteal lymph nodes should demonstrate central fatty change (fatty hilum) on imaging, which is a characteristic feature of benign nodes 2
- Benign lymph nodes typically have smooth, well-defined borders with uniform and homogeneous attenuation 5

Clinical Significance of Palpable Popliteal Nodes
When Popliteal Nodes Are Abnormal
- Any palpable popliteal lymph node warrants further investigation, as these nodes are not normally palpable 1
- In rheumatoid arthritis (a pathologic state), popliteal lymph nodes average 5.4 ± 1.3 mm and are visible in all cases, with multiple nodes present (mean 2.7 nodes) 2
- Loss of central fatty change is concerning and suggests pathology rather than reactive changes 2
Important Clinical Pitfalls
- Popliteal lymph nodes are rarely involved in lymphatic drainage patterns, occurring in less than 10% of patients with lower extremity melanomas 7
- When popliteal nodes are enlarged, consider inflammatory arthropathies (particularly rheumatoid arthritis), malignancy, or infection as potential etiologies 2
- The correlation between popliteal lymph node size and inflammatory synovial volume suggests that larger nodes reflect greater inflammatory burden 2
Practical Assessment Algorithm
On physical examination:
- Popliteal nodes should not be palpable in normal individuals 1
