Dymista Dosing and Duration for Allergic Rhinitis
Dymista should be used at 1 spray per nostril twice daily continuously throughout the entire allergy season (or year-round for perennial allergic rhinitis), not on an as-needed basis. 1, 2
Recommended Dosing
- Standard dose: 1 spray per nostril twice daily for patients ≥12 years of age 1, 2
- Total daily dose: 274 mcg azelastine hydrochloride and 100 mcg fluticasone propionate per day 1
- This dosing applies to both seasonal and perennial allergic rhinitis 1
Duration of Use
Continuous treatment is essential and superior to intermittent use:
- Use Dymista continuously for the entire duration of the allergy season for seasonal allergic rhinitis 2
- For perennial allergic rhinitis, use year-round 2
- Maximal efficacy is reached over days to weeks, making regular continuous use essential rather than intermittent dosing 2
- The evidence clearly demonstrates that continuous daily treatment is more effective than episodic use because ongoing allergen exposure requires sustained medication effect 2
Long-Term Safety
- Dymista has been studied for 52 weeks of continuous use in chronic rhinitis patients with excellent safety and tolerability 3, 4
- The incidence of treatment-related adverse events was low (9.4%) with no evidence of late-occurring adverse events after 1 year of use 3
- No nasal mucosal ulcerations or septal perforations were observed, and adverse findings decreased as treatment progressed 3
- No clinically important differences in fasting AM serum cortisol levels after 12 months of treatment 3
Critical Pitfall to Avoid
Do not use Dymista on an "as-needed" or intermittent basis - this is a common error that significantly reduces efficacy. 2 The combination formulation requires regular twice-daily dosing to maintain therapeutic levels of both the antihistamine and corticosteroid components. 2
Common Side Effects
- Bitter taste (most common, can be minimized with proper technique) 1
- Epistaxis 1
- Somnolence 1
- Headache 1
