From the Research

Treatment of iliotibial band syndrome should prioritize conservative measures, including rest, activity modification, and anti-inflammatory medications, as supported by the most recent and highest quality study 1. The goal of treatment is to reduce pain and inflammation, promote healing, and improve function, ultimately minimizing morbidity, mortality, and enhancing quality of life. Key components of conservative management include:

  • Reducing or avoiding activities that exacerbate symptoms, such as running, especially downhill running and activities involving repetitive knee flexion
  • Using NSAIDs, such as ibuprofen (400-600mg three times daily) or naproxen (500mg twice daily), for 1-2 weeks to reduce pain and inflammation 2
  • Applying ice for 15-20 minutes several times daily, particularly after activity, to reduce pain and inflammation
  • Performing stretching exercises targeting the iliotibial band, hip abductors, and tensor fasciae latae, 2-3 times daily, holding each stretch for 30 seconds and repeating 3-5 times 3
  • Foam rolling the lateral thigh for 1-2 minutes daily to release tension in the IT band
  • Strengthening exercises for hip abductors, particularly the gluteus medius, to address weakness that may contribute to IT band syndrome 4
  • Physical therapy focusing on correcting biomechanical issues, gait analysis, and proper running technique, which is highly effective in promoting recovery 5 Corticosteroid injections may be considered for refractory cases, and footwear assessment and possible orthotic prescription can address contributing factors. Most cases of iliotibial band syndrome resolve within 4-8 weeks with conservative treatment, and surgery is rarely needed, but may be considered for cases that don't respond to 6-12 months of conservative management 1.
