Evidence from Untreated Patients
The strongest evidence comes from quality of life studies comparing untreated men under active surveillance:
Early studies found no differences in general health-related quality of life domains (including vitality/energy) between untreated men with localized prostate cancer and age-matched healthy controls without cancer. 1
Men on watchful waiting or active surveillance do not experience fatigue as a symptom of the cancer itself, though they may develop other issues like erectile dysfunction over time. 1
When Fatigue Does Occur in Prostate Cancer
Fatigue becomes relevant in three specific contexts:
1. Treatment-Related Fatigue
- Both surgery and radiotherapy cause temporary declines in vitality/energy shortly after treatment, with surgically treated men reporting the most dysfunction. 1
- Most men recover baseline energy levels by one year post-treatment. 1
- External beam radiation therapy commonly causes fatigue during treatment, though it is typically temporary and not emotionally distressing to most men. 1

Clinical Implications
If a patient with truly localized prostate cancer (no treatment yet initiated) reports significant fatigue, look for alternative explanations:
- Comorbid psychiatric conditions (depression, anxiety, substance use) 1

- Unrelated medical conditions (anemia, thyroid dysfunction, sleep disorders) 1, 4
- Psychological distress from the cancer diagnosis itself 1
