Side Effects of Oral Minoxidil
Oral minoxidil causes hypertrichosis (unwanted body hair growth) in 15-93% of patients depending on dose, with cardiovascular effects like fluid retention, tachycardia, and lightheadedness occurring far less commonly at low doses (≤5 mg) used for hair loss compared to antihypertensive doses. 1, 2, 3
Dermatologic Side Effects
Hypertrichosis (Unwanted Hair Growth)
- Hypertrichosis is the most common side effect, occurring in 15-17% of patients at doses ≤2.5 mg daily, but can affect up to 93% of patients at 5 mg daily. 1, 2, 3
- The effect is dose-dependent and typically mild at doses ≤2.5 mg daily. 1
- Hypertrichosis is reversible upon discontinuation of the medication. 4
- This side effect led to treatment withdrawal in only 0.5-1.7% of patients in large cohort studies. 2
- Scalp occlusion (wearing tight caps or wigs) can increase systemic absorption and worsen hypertrichosis, even with topical formulations. 5
Other Dermatologic Effects
- Scalp irritation, itching, and redness can occur, particularly with topical formulations. 4
Cardiovascular Side Effects
At Low Doses for Hair Loss (≤5 mg)
- Cardiovascular effects are far less common at hair loss doses compared to antihypertensive doses (5-100 mg). 1
- Lightheadedness occurs in 1.7% of patients. 2
- Fluid retention/edema occurs in 1.3-10% of patients. 2, 3
- Tachycardia (rapid heartbeat) occurs in 0.9% of patients. 2
- Periorbital edema (swelling around eyes) occurs in 0.3% of patients. 2
- No life-threatening adverse effects were observed in a large multicenter study of 1404 patients. 2
Pericardial Effusion
- Pericardial effusion is a rare but serious idiosyncratic complication that requires monitoring. 6, 7
- FDA adverse event reporting detected a significant signal for pericardial effusion even at doses ≤1.25 mg (ROR = 16.41), with the risk substantially higher in patients with pre-existing cardiac impairment (ROR = 49.42 at ≤2.5 mg). 7
- The association with "life-threatening" outcomes was only significant at doses ≤10 mg. 7
At High Doses for Hypertension (5-100 mg)
- Severe sodium and water retention requiring mandatory loop diuretic therapy. 6, 8
- Reflex tachycardia requiring mandatory beta-blocker therapy. 6, 8
- Orthostatic hypotension, particularly in elderly patients. 6
Other Systemic Side Effects
- Headache occurs in 0.4% of patients. 2

Monitoring Recommendations
For Low-Dose Use (Hair Loss)
- Baseline blood pressure and heart rate should be measured before initiating therapy. 1
- Follow-up assessment at 3 months to evaluate tolerance and early response. 1
- Periodic blood pressure checks if dose exceeds 2.5 mg daily. 1
- No requirement for concomitant diuretics or beta-blockers at low doses used for hair loss. 1
For High-Dose Use (Hypertension)
- Supine and upright blood pressure measurements at every follow-up visit to detect orthostatic hypotension. 6
- Cardiac monitoring to screen for pericardial effusion. 6

Contraindications and High-Risk Populations
- Avoid in patients with known pericardial effusion or significant cardiovascular disease. 1

- Pregnancy category C - avoid in women who are pregnant or planning pregnancy. 1
- Not for use in children under 18 years for hair loss indications. 4
- Patients with pre-existing cardiac impairment have substantially higher risk of pericardial effusion. 7
Critical Pitfalls to Avoid
- Do not use high-dose minoxidil (>5 mg) for hair loss without concurrent loop diuretic and beta-blocker, as this combination is mandatory at antihypertensive doses. 6, 8
- Avoid scalp occlusion (tight caps, wigs) during and after application, as this increases systemic absorption and risk of hypertrichosis. 5
- Use the lowest effective dose (≤5 mg/day) to minimize adverse effects, particularly pericardial effusion risk. 7
- Allow 2-4 hours drying time before going to bed after nighttime application to reduce systemic absorption. 4
- Wash hands immediately after application if using fingers to prevent unwanted hair growth on hands. 4
